A later report said a St. Johns County Fire Rescue representative described the emergency call as involving a “nonresponsive person in cardiac arrest.” That is a description of the call—not a confirmed medical examiner finding. The sources available do not establish Rivers’s official cause or manner of death.
What did authorities report about the emergency call?
On October 21, 2025, TMZ reported that a St. Johns County Fire Rescue representative described the call involving Rivers as concerning a “nonresponsive person in cardiac arrest.” TMZ also reported that the final cause was unclear. TMZ’s report is a secondhand account of a representative’s description; it is not a published medical examiner determination.
“Cardiac arrest” in that reported call description should not be treated as proof that cardiac arrest was officially determined to be the cause of death. An emergency-call description conveys the situation responders were sent to address. A final cause and manner determination is a separate medical examiner function.
What did Limp Bizkit announce?
The band announced Rivers’s death on social media in October 2025 but did not disclose the cause or circumstances. The Associated Press reported that he was 48 and died on Saturday, October 18; its account of the band’s announcement contains no cause of death. The Associated Press report, published by CBS News, quoted the band’s tribute: “From the first note we ever played together, Sam brought a light and a rhythm that could never be replaced.” The Guardian likewise reported on October 19 that the cause had not been revealed. The Guardian’s report also did not provide a cause.

Why the call description is not an official cause of death
St. Johns County Medical Examiner District 23 says it determines cause and manner of death using information from law enforcement alongside medicine and science. The county describes cause and manner as distinct determinations; possible manners include natural, accident, suicide, homicide, or undetermined. The county’s Medical Examiner page explains the district’s role, but does not publish a Rivers-specific finding.
The reported call language and an official conclusion therefore answer different questions. The former describes what responders were reportedly told or sent to address. It does not establish the medical examiner’s final finding about what caused the death or how the death is classified.

Is there a final medical examiner report?
The available sources do not provide a case-specific final cause or manner determination for Rivers. St. Johns County says a completed autopsy report typically takes 60–90 days to be completed and finalized, and its contact page explains how reports may be requested. That general county guidance does not confirm that Rivers had an autopsy, that a report was issued, or what any report concluded.
District 23 covers St. Johns, Putnam, and Flagler counties. The county’s general jurisdiction and report information should not be mistaken for a finding in Rivers’s case. No cause should be inferred from unverified claims, reported health history, or the emergency-call wording.
