Likely, the scene in question is Alexandra’s medical examination at Ellis Island in season 2, episode 3, “Wrap Thee in Terror.” The scene draws on a real practice: U.S. Public Health Service doctors inspected arriving immigrants and could refer people for further examination. But the historical evidence does not show that Alexandra’s exact experience, exposure, or dialogue happened to a real immigrant exactly as depicted.
Which scene is the title referring to?
Paramount+ lists “Wrap Thee in Terror” as season 2, episode 3. In a TVLine interview, Julia Schlaepfer, who plays Alexandra, discusses filming the examination sequence. She said it took eight hours to film and was shot on a closed set because of its nudity and violation. That is production testimony, not evidence of the historical examination’s precise details.
What really happened at Ellis Island?
The National Park Service says U.S. Public Health Service doctors conducted medical line inspections at Ellis Island. They looked for signs of contagious disease and other conditions, including physical or mental weakness. Someone suspected of having a health issue could be marked and taken out of line for a further physical or mental examination. That establishes that more thorough examinations existed; it does not mean every immigrant underwent an intimate full-body examination.
The Registry Room served as a site for medical and legal examinations, with the medical inspection first. The National Park Service describes the room as busy and often frightening to new arrivals. Its account helps establish the institutional setting, but it does not document the specific events in the episode.

Does an archive show examinations of women?
Yes. The Library of Congress catalog includes a photograph titled “Physical examination of female immigrants at Ellis Island, New York City.” It corroborates that examinations of female immigrants were documented. The catalog entry, by itself, does not establish the photograph’s exact date or circumstances, the conditions of consent, or that the examination resembled Alexandra’s scene.
How does the 1923 setting fit the timeline?
The story is set just before a policy shift. The National Park Service says the Immigration Act of 1924 enabled prospective immigrants to be inspected before leaving their home country, contributing to a decline in mass processing at Ellis Island. That later change should not be mistaken for the system operating in the episode’s 1923 setting.

What the actor’s account can—and cannot—tell us
Schlaepfer recalled the director’s framing of the examination’s routine nature: “And that’s what was so horrifying about it, because that was their job, and it was so normal.” She was describing the scene’s dramatic effect and direction, not quoting an Ellis Island official or a historical witness.
So the best-supported answer is that the scene is based on the real system of medical inspections and referrals at Ellis Island. The available historical sources do not prove that a real immigrant experienced the scene’s precise degree of exposure, actions, or dialogue.
