The episode opens with Sofia Falcone injured and abandoned after Oz leaves her during the Maroni confrontation. As she drifts through a feverish present, the story returns to the polished philanthropist she once was—and shows how betrayal, her family’s power, and ten years in Arkham helped turn her into the woman who now chooses violence on her own terms.
What happens in “Cent’anni”?
“Cent’anni” is Sofia’s backstory episode, framed by the aftermath of Oz leaving her behind. Its central contrast is stark: in the past, Sofia tries to preserve her mother’s memory and expose what she believes Carmine Falcone has done; in the present, she decides that the family which helped destroy her will answer to her.
The episode does not present her final actions as an escape from violence. Sofia protects a child, then kills most of the Falcone household and spares Johnny Viti for a confrontation. Her history explains her rage, but does not make the massacre someone else’s decision.
How Sofia’s questions about Carmine became a case against her
The foundation and the reporter
Before Arkham, Sofia runs a foundation named for her mother and travels with Oz as her driver. Reporter Summer Gleeson asks about women found hanged at businesses connected to Carmine. The questions strike a personal nerve: Sofia remembers finding her mother’s body.

At dinner, Carmine suggests Sofia take over the family business. When she tries to talk about her mother, he shuts the conversation down. Summer later shows Sofia evidence of defensive wounds that, in her view, does not fit simple suicides. Sofia confronts Carmine, pointing to her suspicions and scratches on his hands. He dismisses her as confused and sick.
Oz’s betrayal and Sofia’s arrest
Oz tells Carmine that Sofia met with Summer. That betrayal gives Carmine’s account a path into the authorities’ hands. Police arrest Sofia and charge her with the women’s murders, including Summer’s. Her questions about Carmine and her mother are recast as evidence against her, while the Falcone family’s support for Carmine helps keep her confined.

What happened to Sofia in Arkham?
The episode depicts Sofia spending ten years in Arkham amid abuse, coercion, and institutional control. When she learns that her confinement will continue longer than she expected, she violently attacks another patient. This is the show’s fictional depiction of Sofia’s experience, not a clinical account of real-world psychiatric care.
Her time there compounds the original betrayal: people with power decide what she is, and her family’s version of events helps make that judgment stick. The showrunner Lauren LeFranc has connected the story to histories in which women were labeled insane or confined as hysterical. That is the creator’s framing of the drama’s gendered power struggle, not evidence that the series accurately represents psychiatric practice.

Why does Sofia kill her family?
In the present, Dr. Julian Rush treats Sofia after Victor helps Oz escape the Maronis. She tells Rush she feels foolish for trusting Oz and challenges his role in her past. Rather than accept another person’s judgment that she is sick, Sofia decides she is done being told who she is.
At a family dinner, she confronts her relatives over their part in her confinement and toasts to a new life. She gets her cousin’s young daughter safely into the greenhouse before releasing gas through the mansion. The child survives; most of the Falcone household does not. Johnny Viti is spared because his window is open. Sofia wakes him at gunpoint and tells him they need to talk.

That ending is not a clean break from her family or the violence that shaped her. It is a forced negotiation on Sofia’s terms. She has gone from being spoken for to deciding who lives, who dies, and who must answer her.

The episode answers how the “Hangman” reputation was attached to Sofia, then shows her becoming capable of the violence people attributed to her. Both parts matter. Carmine’s power and Oz’s betrayal help explain how she was framed and confined; her Arkham experience shows what prolonged control does to her trust and sense of agency. But the massacre remains her choice.

Cristin Milioti described the transformation to TV Insider this way: “What I love about that fourth episode is that you get to see how she was driven mad. She has become the thing that she was accused of being by way of what they did to her.” The line captures the episode’s bitter irony, not an absolution. Asked by TheWrap whether Sofia’s rage was always there or was changed by Arkham, Milioti answered: “I think it’s both. I don’t think even she knows. I think it’s both.”
Director Helen Shaver told TV Insider that when she reached the episode’s script, she knew she wanted to direct it: “I got to [Episode 4]. I didn’t even have to look at [Episode 5]. I said, ‘I’m doing it. This is a great script. I want to tell this story.’” The episode’s structure makes that story a reversal: the composed public figure becomes the target of a family conspiracy, then returns as the person imposing the terms of the next conversation.
