The Old Guard never gives one definite reason Andy loses her immortality. The change is clear on screen: her wounds stop healing, leaving her mortal. Netflix’s explanation offers two possibilities—she may have exhausted her immortality, or it may be fading with age—but confirms neither as the cause.
What happens to Andy?
For most of the film, Andy’s injuries heal, making her effectively immortal. Near the end, she is wounded and discovers that her body no longer regenerates as it once did. The important change is not a new power or a temporary setback: Andy has become vulnerable to death.

Why does her immortality stop?
The film leaves the mechanism unexplained. In its ending explainer, Netflix presents two possible explanations: Andy may have used up her immortality, or it may be fading as she ages. These are possibilities, not a definitive diagnosis established by the story.
The loss is at least possible within the film’s rules. Netflix’s explainer points to an earlier flashback in which an immortal has lost the ability to regenerate. That precedent makes Andy’s change consistent with the film’s world, but it does not reveal exactly why it happens to her.

Does The Old Guard 2 explain it?
No definitive cause is supplied in Netflix’s official description of the sequel. That description confirms the consequence: Andy has lost her immortality and is mortal. It establishes her changed status, not whether age or exhaustion caused it.

The first film is based on the graphic novel by Greg Rucka and Leandro Fernández. An Esquire comparison describes Andy’s loss of immortality as a major difference from the comics, so the reveal is best understood as part of the film adaptation’s story rather than as an explanation drawn from the source material. Director Gina Prince-Bythewood has also discussed the idea of Andy being on her “last life”; that provides adaptation context, but does not settle the fictional cause.

Readers interested in that difference can compare the film with Rucka and Fernández’s The Old Guard graphic novel. The comic is useful for seeing how the adaptation changes Andy’s arc, not for resolving the film’s unanswered in-universe question.
