Spoilers for Season 7, Episode 10, “Goodbye.” Glassman dies after his brain cancer returns, while Claire survives a life-threatening infection but loses her left arm. The finale then jumps forward several years to show Shaun leading surgery at St. Bonaventure and building a life shaped by what Glassman taught him.
What happens to Glassman in the finale?
Glassman tells Shaun his brain cancer has returned and is terminal. Shaun responds by trying to find a treatment, approaching the news as a medical problem he can solve. Glassman refuses further treatment: he wants to spend the time he has left with Shaun and the people he loves.
Shaun eventually accepts what Glassman is asking of him and promises to be present. They ride the carousel Glassman once shared with his late daughter, Maddie. Later, the episode shows Shaun alone on the carousel. Glassman’s death is conveyed by that cut rather than by showing his final moments.
Does Claire survive?
Claire returns to St. Bonaventure, where an aggressive infection puts her life at risk. Shaun pursues a non-FDA-approved treatment, willing to jeopardize his medical license to try to save her. Claire survives, but the infection costs her her left arm. Glassman intervenes when Shaun is close to giving up his career.

Where are Shaun and the others after the time jump?
The finale jumps ahead several years. Shaun is chief of surgery at San Jose St. Bonaventure Hospital. In a TED Talk, he reflects on his life, his work and Glassman’s influence. The ending returns to the series’ opening idea: Shaun wanted to help people and save lives. He adds a joke about now having two televisions.

Six months after Glassman’s death, Shaun and Claire founded the Dr. Aaron Glassman Foundation for Neurodiversity in Medicine. TVLine’s 2024 finale recap reports that the fictional foundation has 17 hospital affiliations and 32 medical-school affiliations; those are story details, not verified figures for a real organization.
What does the ending mean for Shaun?
Shaun’s central challenge is not finding the right medical intervention. Glassman does not want treatment; he wants a personal relationship with Shaun during his remaining time. Shaun’s decision to stop trying to solve the illness and instead honor Glassman’s wishes marks a change in how he responds when someone he loves needs something medicine cannot provide.

Co-showrunner Liz Friedman described the conflict in TVLine’s May 21, 2024 interview: “Shaun faces a really unique challenge because of his ASD: He’s a doctor, and his mentor has this very serious illness. He wants to deal with it as medicine, and Glassman doesn’t want to do that; he simply wants to have a personal relationship.”

The boardroom scene echoes Shaun’s pilot-era appeal to the hospital board. Freddie Highmore said returning to that room put Shaun’s journey into perspective, while co-creator David Shore said it helped bookend the series. In the finale, Shaun is no longer asking the board to give him a chance: he has become chief of surgery and is responsible for leading others.
