How the characters connect
Most of the film’s relationships run through the Vizzardeli Orphanage. These are the links that drive the plot:
- Margaret and Carlita: both are orphans, and Margaret’s bond with the outcast Carlita sets up much of her emotional arc.
- Margaret and Luz: the roommates are presented as contrasting figures, and the production notes describe Luz as a foil to Margaret.
- Gabriel and Cardinal Lawrence: the young priest is mentored by the senior cardinal, a relationship that becomes important as the church hierarchy comes into focus.
- Brennan and Margaret: Brennan seeks Margaret out to warn her about a church conspiracy, placing him outside the orphanage’s day-to-day world.
- Silva and the novitiates: as abbess, Silva oversees the nuns and novitiates, including Margaret and Luz.
Setting and premise
The story is set in 1971. After she is sent to Rome for church service, Margaret encounters a darkness that makes her question her faith. The official synopsis says she uncovers a conspiracy that aims to bring about the birth of evil incarnate. The production notes give the Vizzardeli Orphanage a population of 62 girls.

The film is positioned as a prequel to the Omen franchise, and its director and co-screenwriter, Arkasha Stevenson, has described its relationship to the earlier film: “The First Omen is a prequel to the 1976 version, but what was also really important to us was that we create a story that could stand on its own.” Viewers do not need to have seen the 1976 film to follow the 2024 story.

The rating and runtime differ by market. 20th Century Studios lists the U.S. release date as April 5, 2024. Disney+’s UK catalog uses different figures, so check the listing for your country.

The studio’s film page lists the title on Blu-ray, DVD and digital. Disney+ also lists the film in the UK, but its catalog warns that availability can vary by region. Streaming access in any other country is not established by these sources, so confirm the current listing in your market before you buy or subscribe.
The 2024 date and the ratings above are the values reported by the studio and by Disney+ UK. They are not universal across markets.
