Spoilers for 28 Years Later (2025). The train scene is the abandoned-carriage encounter with an infected woman in labor; it is not Isla’s death. Isla dies later, after the family reaches Dr. Ian Kelson.
What happens in the train scene?
Spike, Isla, and Erik hear a sound coming from an abandoned train carriage. Isla goes to investigate. The scene leads to an infected woman who is in labor, turning what begins as a disturbing noise into an encounter with childbirth. Sound supervisor Johnnie Burn describes the sequence as a progression through the carriage to that discovery in his interview with A Sound Effect.

The scene is staged around what the characters hear before they understand what is happening. That sound-led approach gives the audience and Isla a reason to move toward the carriage without first explaining the threat visually. Burn’s account supports that description of the scene’s sound design; it does not provide a complete transcript or shot-by-shot account.
Does Isla die in the train scene?
No. The infected woman’s childbirth and Isla’s death are separate events. The train encounter occurs while Isla is still with Spike and Erik. Later, after the family reaches Dr. Ian Kelson, Isla’s condition worsens, and Kelson helps her die peacefully, according to Netflix Tudum’s ending explainer.

Why does Isla investigate the sound?
Isla’s decision to investigate gives the scene its immediate movement: she responds to the sound rather than staying at a distance. In an interview with VPM/NPR, director Danny Boyle discusses the train encounter in connection with Isla’s illness and Spike’s search for a doctor. Read together, the moment can be understood as part of the film’s attention to Isla’s vulnerability and the family’s circumstances, rather than as a scene that explains everything about the infected.
What the scene suggests about the infected
The childbirth encounter complicates a simple view of the infected as only a source of danger: the woman is still in the midst of a human experience, even as the people who find her face a frightening and uncertain situation. That is an interpretation of the scene, not a definitive statement of Boyle’s intended message. The director interview and Burn’s sound-design discussion establish the encounter and its staging, but do not supply a complete dialogue transcript or a definitive explanation of its meaning.
