The ending leaves Sienna searching for her cousin and Art still alive—a deliberately unresolved finish that Art actor David Howard Thornton said leaves both characters vulnerable.
What happens at the end of Terrifier 3?
Art and Vicky lure Sienna to the family home and restrain her. Their plan is not simply to kill her: the demon acting through Vicky attempts to possess her. Sienna resists, and the sword she had hidden as a present becomes her way to fight back.

Art and Vicky torment Sienna with what appears to be Gabbie’s severed head. The reveal is that the head belongs to Jonathan. Gabbie is alive, and she helps Sienna reach the sword. Sienna decapitates Vicky and wounds Art with it, but the fight does not end in a clean victory.
Vicky’s body is drawn back toward the underworld. As Sienna tries to save Gabbie, Gabbie falls through the portal. Sienna is left grieving and promising to find her cousin. Art, though injured and pinned during the struggle, gets away; in the final scene, he boards a bus beside another passenger.

Does Art die?
No. Sienna wounds Art with the sword, but he escapes and is still a threat at the end of the film. The cliffhanger leaves his conflict with Sienna unresolved.
Whose head does Art show Sienna?
It is Jonathan’s head, not Gabbie’s. Gabbie is alive when the portal takes her, so Sienna’s immediate goal is to find and rescue her cousin.

What David Howard Thornton said about the cliffhanger
Thornton, who plays Art the Clown, said the ending was the moment he was most excited about because it avoids a happy, tidy resolution. He described Sienna and Art as being in “very vulnerable states of being.” In context, the parallel is emotional and dramatic: Sienna has lost Gabbie to the portal, while Art has lost Vicky and is wounded. Thornton was not saying that the two are equally powerful or in identical circumstances. CBR’s interview with Thornton
He also said the unresolved ending created possibilities for the next story, including exploring Art’s mental state and how he responds to loss. Those comments describe Thornton’s hopes for future storytelling; they do not establish what a later film’s plot will be.

In a later appearance reported by Popverse, Thornton said the ending went through multiple drafts and was originally longer and “much more dark.” He said it was condensed for time and that he urged director Damien Leone not to change it. He compared the unresolved finish to an “Empire Strikes Back” ending. Popverse’s report on Thornton’s remarks

- Sienna can resist the attempted possession. Vicky’s effort to possess her fails.
- The sword can harm the supernatural threat. Sienna uses it to kill possessed Vicky and wound Art.
- Jonathan is dead, but Gabbie is alive when the portal takes her. Sienna’s attempt to save her cousin is unresolved.
- Art survives and escapes. The film does not settle what he will do next.
The finale frames Sienna and the demon’s forces as part of a larger supernatural struggle, but it does not spell out every rule governing the sword, Art, or the portal. The ending alone does not settle Art’s full origin or explain everything about the demon. Any claim about what a future installment will reveal should be treated as speculation unless attributed to a named source.
