In the final confrontation, Smoke appears to kill Stack. He then kills Remmick before Remmick can turn Sammie. Smoke sends Sammie home, but stays behind when members of the Ku Klux Klan return to attack the club. Smoke kills the attackers, but is mortally wounded.
The film circles back to its opening church sequence: Sammie arrives bloodied, carrying his damaged guitar, then leaves with it to begin a life in music.

What happens in the mid-credits scene?
Stack and Mary return in 1992
The mid-credits scene jumps forward to 1992. Older Sammie, played by blues musician Buddy Guy, is performing at his own club when Stack and Mary arrive. They appear unchanged by age because they are vampires.
Smoke spared Stack
Stack tells Sammie that Smoke spared him after the brothers fought. Smoke made Stack promise to stay away from Sammie, which explains why Stack did not return sooner.

Sammie refuses immortality
Stack offers to turn Sammie so he can live longer without pain. Sammie declines. The two remember the juke joint’s opening night as a joyful, freeing experience, even though it ended in terrible violence.
What is the final post-credits scene?
The last scene returns to young Sammie in church. He plays the twins’ guitar and sings “This Little Light of Mine.” That is what the scene shows; whether he is looking at a supernatural presence is open to interpretation, not confirmed plot fact.

What the ending suggests about Sammie and Stack
The mid-credits reveal changes the apparent finality of Smoke’s fight with Stack: Stack lived, but his survival meant becoming a vampire and living under the limits that come with it. The reunion also gives the brothers’ last shared night a different emotional weight. In their memory, the opening celebration was a moment of freedom, not only the prelude to the horror that followed.
Sammie’s refusal sets up a contrast. Stack can continue living, but Sammie chooses a mortal life and the chance to make music. The church scene works as a coda to that choice: rather than ending on the vampire world, the film returns to the young musician and the path he takes after the massacre.

Why music matters to the finale
The blues is more than the talent that draws people to Sammie’s performance. In an interview with The Atlantic, writer-director Ryan Coogler said the central performance sequence let him express the blues’ reach across time: “I had to express, in a way that only film language can express, the monumental importance of what these people are doing and how it would reach through time.”
Coogler also said, “The vampires were always there, because so much music deals with the supernatural.” In that context, the return to Sammie singing in church connects the film’s supernatural violence with its longer view of music, memory, and what can endure.
