Two documentaries released in 2025 examine the Titan submersible disaster: Netflix’s Titan: The OceanGate Submersible Disaster, a film, and Disney+’s Implosion: The Titanic Sub Disaster, a series. For the official investigative conclusion, look beyond either documentary to the U.S. Coast Guard Marine Board of Investigation’s final report, released August 5, 2025. It called the tragedy preventable and identified OceanGate’s failure to follow established engineering protocols for safety, testing, and maintenance as the primary causal factor.
What happened to the Titan submersible?
On June 18, 2023, the commercial submersible TITAN imploded during an expedition to the Titanic wreck. All five people aboard died. The U.S. Coast Guard Marine Board of Investigation’s final report describes it as “a preventable tragedy.” The Coast Guard’s report announcement identifies the failures in OceanGate’s established engineering protocols for safety, testing, and maintenance as the primary causal factor.

What did the Coast Guard investigation conclude?
The final Marine Board report was released on August 5, 2025. Its conclusion gives readers a more authoritative basis for understanding the disaster than a documentary’s account or reconstruction: investigators identified OceanGate’s failures to follow established engineering protocols for safety, testing, and maintenance as the primary causal factor. The report’s findings are the Coast Guard’s investigative conclusions; a documentary may present those findings alongside interviews, archival material, or its own narrative choices.
Netflix Tudum’s June 11, 2025 explainer discussed the documentary’s treatment of the hull design, acoustic monitoring, and warnings. At the time, it accurately said the Coast Guard report had not yet been released. That status changed with the report’s publication on August 5, 2025, so the June account is useful for understanding the film’s framing, not for the current status of the investigation.
