What happened aboard the Aleutian Lady
Meadows, 25, was a deckhand aboard the crab-fishing vessel Aleutian Lady. According to the Coast Guard account reported by the Associated Press on March 3, 2026, the Coast Guard received notice shortly after 5 p.m. on Feb. 25 that he had fallen overboard about 170 miles (274 kilometers) north of Dutch Harbor, Alaska.

Coast Guard spokesperson Chief Petty Officer Travis Magee said, “He was recovered unresponsive by the crew approximately ten minutes later,” and that resuscitation efforts were unsuccessful. The crew brought Meadows’ body to Dutch Harbor.
What the death certificate said
The Los Angeles Times reported that the certificate, which TMZ obtained, listed drowning with probable hypothermia and submersion of the body in cold water. The Times said the Alaska Department of Health ruled the death an accident.

Those findings do not establish why Meadows fell overboard or describe the circumstances of the fall beyond the reported overboard event. The certificate’s cause-of-death finding should not be read as an eyewitness account.
What has been reported about the show’s footage
The Times reported that Meadows’ mother, Angela Meadows, asked Discovery not to broadcast footage of her son’s death. It also reported that production was reportedly wrapping season 22 and cameras were rolling when the incident occurred. The presence of cameras does not establish that the fatal incident was filmed or aired.
A TMZ listing updated September 27, 2026, says the season finale aired Meadows’ final call with his children. The listing does not provide enough context to characterize the segment or establish that the episode showed the fatal incident.

The Coast Guard said it was investigating in the AP’s March 3 report. The available later listing does not confirm whether that investigation has concluded, so its status as of October 8, 2026, is not established here.

Discovery said, “This is a devastating loss, and our hearts are with his loved ones, his crewmates, and the entire fishing community during this incredibly difficult time.” Captain Rick Shelford said, “We lost our brother,” according to the AP.
