Music Engagement Is Linked With Lower Dementia Risk in Older Adults

A large study of Australians aged 70 and older linked music listening and instrument playing with lower dementia risk. The findings do not prove music prevents dementia or establish a preventive dose.

Listening to music and playing an instrument were associated with lower dementia risk in a large study of Australians aged 70 and older—but the study does not show that music prevents dementia. The findings are encouraging, not a medical guarantee, and they do not establish how much music someone would need to hear or play to change their risk.

Does listening to music prevent dementia?

It has not been shown to prevent dementia. A 2025 prospective cohort study found that older adults who reported always listening to music had a lower rate of dementia during follow-up than those who listened never, rarely, or sometimes. Because participants were not randomly assigned to listen to music, the result is an association, not proof that listening caused the difference.

The study analyzed data from 10,893 community-dwelling Australians aged 70 and older who did not have dementia when recruited. Researchers adjusted their models for age, gender, and education, and dementia-risk analyses began from the third year. Even with those adjustments, other differences between participants could help explain the result.

Always listening, compared with never, rarely, or sometimes, was associated with a 39% lower relative risk of dementia (95% confidence interval 0.45–0.82; p=0.001). This is not a 39-percentage-point reduction in an individual’s chance of developing dementia. The study’s authors described music as a potential accessible strategy for cognitive health, while noting that their observational design could not establish causation. Read the 2025 cohort study.

Can playing an instrument lower dementia risk?

The same cohort found that often or always playing an instrument was associated with a 35% lower relative dementia risk than less frequent playing (95% CI 0.42–0.99; p=0.047). The estimate was close to the conventional statistical threshold and less precise than the listening estimate. It should be treated as suggestive, not as evidence that taking up an instrument will prevent dementia.

A 2022 systematic review and meta-analysis of three prospective cohorts—two from the United States and one from Japan—also found an inverse association. Instrument players had a pooled hazard ratio of 0.64 for dementia (95% CI 0.41–0.98). The small number of observational cohorts and the possibility of residual confounding limit what can be concluded: the analysis strengthens the case for a possible link, but does not establish a protective causal effect. What did the study find about listening and playing together?

Participants who engaged in both listening and playing had a 33% lower relative dementia risk than the comparison group in the 2025 study (95% CI 0.51–0.89; p=0.006). The combined activity category was also associated with a 22% lower risk of cognitive impairment without dementia. The study does not show that combining activities is better than either activity alone; the categories and comparisons do not establish which activity, if any, is most effective.

For additional context, always listening was associated with a 17% lower risk of cognitive impairment without dementia. These are observational associations with different outcomes from incident dementia, not evidence that music improved participants’ subjective cognitive wellbeing; the paper reported no significant association with changes in that measure.

How should the different kinds of music research be read?

Studies of people who already have dementia address care and symptoms, not whether music prevents dementia in people who do not have it. They should not be combined with cohort studies of new dementia diagnoses as though they answer the same question.

Music therapy for people with dementia

A 2025 Cochrane review included 30 randomized studies with 1,720 people who already had dementia; its evidence search ran through November 30, 2023. Compared with usual care, music-based therapeutic interventions probably produced a slight improvement in depressive symptoms at the end of treatment (standardized mean difference −0.23; 95% CI −0.42 to −0.04; moderate-certainty evidence). They may also have improved overall behavioral problems, based on low-certainty evidence. The review did not find evidence of cognitive benefit versus usual care or establish lasting effects. These are findings about support for people with dementia, not prevention. Read the Cochrane review.

Active music-making and cognitive test performance

A 2021 meta-analysis of nine randomized studies involving 495 older adults with probable mild cognitive impairment or mild-to-moderate dementia found a small effect on cognitive functioning from active music-making (standardized mean difference 0.30; 95% CI 0.10–0.51). Participants already had impairment, and the outcome was cognitive functioning—not whether they later developed dementia. See the 2021 review.

Is there a recommended amount of music?

No preventive dose is established by these studies. They do not support a specific number of minutes, sessions, years of practice, instrument, or lesson schedule as a way to prevent dementia. The 2025 cohort’s frequency categories are observations about participants’ reported habits, not a tested prescription.

If you enjoy music, listening or playing can be worthwhile leisure activities. Choose what is accessible and enjoyable rather than treating it as a medical regimen. The evidence does not establish that singing, lessons, or buying a particular instrument prevents dementia.

What the evidence can—and cannot—say

  • It can say: In observational studies, some older adults’ music listening and instrument playing were associated with lower dementia risk.
  • It cannot say: Music caused the lower risk, guarantees protection, or has a proven preventive dose.
  • It also cannot establish: That one type of music engagement is superior as a dementia-prevention method, or that intervention findings in people with diagnosed dementia apply to prevention.
