Monster Ending Explained: What Happens to Johan?

Tenma is cleared, the survivors move forward, and Johan remains asleep when Tenma visits. The anime leaves Johan’s ultimate fate unresolved.

Monster ends with Tenma cleared after the Ruhenheim killings, survivors trying to rebuild their lives, and Johan alive but still asleep when Tenma visits him. The anime does not confirm what ultimately happens to Johan. Its closing image leaves that question open while bringing Tenma back to the moral choice that began the story: whether a life is worth saving regardless of status or reputation.

What happens at the end of Monster?

After the events in Ruhenheim, suspicion that Tenma committed serial murders is cleared, and the people who survived begin new lives. Tenma continues working abroad with Doctors Without Borders. He also searches for the twins’ mother, returning repeatedly until he finally hears her account. Then he visits Johan, who survived but remains asleep.

That sequence is the confirmed plot resolution given by the official Japanese synopsis for the final episode. It offers some closure for Tenma and the survivors, but it does not explain Johan’s ultimate fate.

Did Johan escape?

The episode does not establish that Johan escaped, died, or woke up. The official synopsis ends with Tenma visiting him while he is asleep; it does not describe what happens afterward. The often-discussed image of Johan’s empty hospital bed should therefore be treated as unresolved, not as proof of a particular outcome.

Both are interpretations of an image, not settled answers. The ending withholds the information needed to choose between them.

Why does Tenma return to Johan?

Tenma’s visit echoes the decision that set the story in motion. At the hospital, leadership wanted him to prioritize the mayor over the young patient who had arrived first. Tenma refused and saved the child, Johan. His later return to Johan’s bedside recalls that choice: Tenma continues to treat a life as worthy of care without making its value depend on rank, public standing, or what others believe about the person.

This is a structural reading of the ending, not a separate statement of authorial intent. The official English episode title, “The Real Monster,” invites questions about identity and responsibility, but the title does not name any one character as the answer.

Who is the real monster?

The story does not provide an official answer by declaring one character to be “the real monster.” A more grounded way to read its central moral question is through Tenma’s refusal to rank human lives by status or reputation. Johan’s actions make him a source of horror, but the ending’s return to Tenma’s original decision keeps the larger question focused on how people judge—and value—another person’s life.

How does the anime ending relate to the manga?

The anime adapts Naoki Urasawa’s manga; Nippon TV lists it as a 74-episode Madhouse production, and VIZ identifies Urasawa as the manga’s creator.
