The records support calling parts of the ketamine chain around Matthew Perry deeply troubling—but “sketchy beyond absolute belief” is an editorial judgment, not a court finding. Five defendants were ultimately sentenced for their roles, which ranged from unlawful distribution to supplying the ketamine that federal authorities say was administered shortly before Perry died. Their roles were not interchangeable.
What the medical examiner said caused Perry’s death
The Los Angeles County Department of Medical Examiner determined that Perry died from the acute effects of ketamine. The manner of death was ruled an accident. Drowning, coronary artery disease and the effects of buprenorphine were listed as contributing factors.
Postmortem testing found ketamine concentrations of 3,540 ng/ml in peripheral blood and 3,271 ng/ml in central blood. Those are case-specific toxicology results, not a typical dose or medical guidance. The report also recorded buprenorphine at a therapeutic level and lorazepam at a non-toxic level; it detected no alcohol, methamphetamine, cocaine, heroin, PCP or fentanyl.
How ketamine reached Perry
The medical examiner’s report says Perry had received ketamine infusion therapy for depression and anxiety. His last known treatment was about a week before his death, however, and the medical examiner said that treatment could not account for the ketamine present at death.

The federal case concerned a separate chain of distribution and injections. According to the Department of Justice (DOJ), physicians Salvador Plasencia and Mark Chavez were involved in earlier ketamine transfers and treatment. In October 2023, DOJ says, Jasveen Sangha and Erik Fleming sold 51 vials to Perry through his assistant, Kenneth Iwamasa. The Drug Enforcement Administration (DEA) says Iwamasa administered at least three shots of ketamine obtained from Sangha on October 28, the day Perry died, and that those shots caused his death.
That account distinguishes supervised treatment from the conduct prosecuted in this case: the source and purpose of the supply, whether a qualified healthcare professional was supervising administration, and the acute outcome. DOJ says Iwamasa had no medical training and was administering ketamine without medical supervision. The case does not establish that medically supervised ketamine treatment caused Perry’s death.

The DEA described Iwamasa as the fifth and final defendant sentenced on May 27, 2026. The sentences reflect different roles and convictions; they should not be read as though every defendant supplied the fatal ketamine or carried out the same conduct.
What makes the conduct troubling—and what the phrase does not mean
The strongest basis for the article’s “sketchy” characterization is not simply that Perry used ketamine. DOJ’s account describes transfers it says lacked a legitimate medical purpose and fell below the proper standard of care, followed by injections administered without medical training or supervision. The medical examiner’s findings establish an accidental death from acute ketamine effects, while the DOJ and DEA releases describe the prosecution’s account of the supply chain and the defendants’ later pleas and sentences.

In its 2025 sentencing release, DOJ attributed to a sentencing memorandum a text Plasencia sent a co-conspirator on the day he met Perry: “I wonder how much this moron will pay” and “let’s find out.” That is an account of a text cited by prosecutors, not a judicial phrase defining the case. Similarly, DEA Administrator Anne Milgram called the doctors “unscrupulous” when charges were announced on August 15, 2024, and described a path from doctors to street dealers. That was Milgram’s public characterization at the time—not a court finding about every defendant.
The difference in the case record matters: Plasencia admitted to distribution offenses and received a prison sentence, but DOJ says he did not provide the fatal ketamine. Federal authorities tie the fatal supply to Sangha through Fleming, with Iwamasa administering the injections. “Sketchy beyond absolute belief” is therefore a forceful editorial summary of the misconduct described in the record, not a precise legal conclusion about all five people.

What the prosecution established—and what it did not
Federal authorities announced charges against five defendants on August 15, 2024. At that point, DOJ stated that an indictment was an allegation and that defendants were presumed innocent unless proven guilty beyond a reasonable doubt. Later guilty pleas and sentencings changed the case’s posture: by May 27, 2026, all five defendants had been sentenced, according to the DEA.
The official releases summarize the charges, admissions and outcomes, but they do not make the underlying distribution chain a general finding about ketamine treatment. The medical examiner recognized ketamine’s established medical and surgical uses; the fatal case involved the distinct supply and unsupervised injections described above.
