Why the show’s answer seems to change
The story unfolds in stages: Chuck first believes Elizabeth is his mother, Elizabeth later denies it, and Diana then offers a competing account. The season 5 evidence points back toward Elizabeth, though it leaves a question about why she denied the connection.

Season 3: Elizabeth is introduced, then denies being his mother
In season 3, episode 14, Chuck identifies Elizabeth Fisher—formerly known as Evelyn—as his mother. He believes Bart told him she had died and suspects she has returned for money. That episode establishes the mystery; it does not settle it. In a later season 3 episode, Elizabeth tells him, “Chuck, I’m not your mother. I don’t know if your real mother is alive or not.”
Season 5: Diana claims she gave birth to Chuck
In season 5, episode 20, Diana says she had an affair with Bart and became pregnant while he was with Elizabeth. Diana claims Elizabeth could not get pregnant, agreed to raise the child, and later could not continue raising someone else’s child. Her account distinguishes the person who gave birth from the person who raised Chuck; it is Diana’s claim, not the story’s final confirmation.

Season 5: Elizabeth’s photograph challenges Diana’s account
In episode 21, Nate tells Chuck that Elizabeth emailed him a photograph dated just before Chuck’s birth. Chuck points out that this conflicts with Diana’s claim that Elizabeth could not get pregnant. Nate suggests Jack may have prompted Diana’s story and says Jack was funding her. The photograph functions as evidence within the plot, not as an independently authenticated record or medical test.

The eventual direction of the storyline is that Elizabeth is Chuck’s biological mother. A 2010 Los Angeles Times recap reported that Elizabeth did turn out to be the mother Chuck had believed dead, while a 2012 TVLine recap describes Diana’s later claim. The episodes provide the chronology behind that broad conclusion.

There is still an unresolved point in the season 5 exchange: after seeing the photograph, Chuck asks why Elizabeth denied being the blood donor if she really is his mother. The scene points toward Elizabeth without supplying a DNA result or fully explaining the motive behind her denial.
