The clearest step-by-step transformation is a playable system in Fallout 76—not a definitive biological account for every game or the television series.
Are there official stages of ghoul transformation?
No universal sequence, exposure threshold, or transformation timeline is established in the official material cited here. Bethesda’s franchise overview calls ghouls “humans twisted by radiation (and chems)” and says that, in the games, they range “from wise survivors to terrifying feral monsters.” That is a broad description, not a numbered biological process. Bethesda’s franchise overview also connects ghoul portrayals across the show and games without laying out a shared transformation schedule.
A Bethesda guide to playing a ghoul in Fallout 76 describes a ghoul as someone exposed to an inordinate amount of radiation and notes that many familiar ghouls underwent the mutation during the Great War. It does not specify how much exposure causes ghoulification or how long the change takes. A character’s appearance or story may suggest change in a particular scene, but that should not be mistaken for a franchise-wide stage system.
How does becoming a ghoul work in Fallout 76?
Fallout 76 added a player-facing transformation in the Ghoul Within update, released March 18, 2025. Its “Leap of Faith” questline completes the change for the player character and unlocks ghoul-exclusive abilities and perks. These are the game’s rules for that character, not a universal explanation of ghoul biology.

Bethesda’s March 18, 2025 release notes describe how those systems work. The update’s mechanics can change with later patches, so the release notes are the reference for the system as documented at launch.
Are Fallout ghouls immune to radiation?
Not as a blanket rule across every Fallout story. The specific, well-documented case is the playable ghoul in Fallout 76: radiation has a different, beneficial role there. Instead of functioning only as a hazard, accumulated radiation contributes to Glow, which can increase maximum health and heal damage. Bethesda’s support page states: “As a Ghoul, radiation makes you glow. You’re immune to diseases and cannot be mutated by nearby radiation.” Bethesda Support’s explanation of ghoul Glow describes that player-character mechanic.

That means “radiation immunity” is an incomplete shorthand for the Fallout 76 system. Radiation still affects the character by building Glow and interacting with health and perks; it does not simply have no effect. The game’s guide also describes ghouls as able to withstand high wasteland radiation and heal from it, but those claims should remain attached to that game’s depiction rather than generalized to all continuities.

The official sources establish that Fallout includes both sentient or wise ghouls and feral ones, but they do not establish one universal cause or timetable for feralization. In Fallout 76, ferality is represented by a Feral meter: it declines over time and can be restored by consuming Chems. Low meter levels impose penalties; at zero, the character gains a major melee-damage benefit while suffering severe Charisma and accuracy reductions. This is a game mechanic, not proof of a common schedule by which all ghouls lose their minds.
