Elf on the Shelf at the Hospital: 12 Simple, Ward-Safe Ideas

Simple, low-effort Elf on the Shelf scenes for a hospital stay—plus what to ask the care team before bringing or placing anything.

Yes—an Elf on the Shelf can bring a little holiday mischief to a hospital stay, but ask the care team before bringing or placing one. Rules vary by ward, and decorations can raise infection-control or fire-safety concerns. These simple ideas are designed to be enjoyed from bed and adapted to staff guidance, the child’s energy, and their preferences.

Start with the ward’s rules

Hospitals do stage holiday elves: Johns Hopkins Medicine reported that nurses at its Children’s Center place an elf around the units each year, sometimes hanging it from the ceiling or staging playful mischief. That does not mean every hospital permits the same setup. Great Ormond Street Hospital advises families to ask the ward team which decorations are allowed and where they may go, to avoid infection and fire risks. Check before bringing an elf, attaching anything, or sharing supplies.

  • Ask where an elf may safely sit, and whether it can stay in the room overnight.
  • Keep it away from medical equipment, supplies, food, clinical surfaces, and places staff need to reach.
  • Do not let it touch the child, lines, dressings, or devices.
  • If the child is in isolation or has special precautions, follow the team’s directions; skip shared items if advised.
  • Ask separately before arranging outside visitors or group activities. Cincinnati Children’s says special visitor requests are reviewed for safety, infection prevention, appropriateness, and scheduling, and approval is not guaranteed.

Elf on the Shelf ideas for a hospital stay

These are adaptable suggestions, not a list of activities approved by every hospital. Use only a location and materials the care team permits; most can be enjoyed without the child needing to sit up or handle anything.

  1. A bedside-book visitor: Place the elf beside a holiday or favorite book on an approved table or shelf. A caregiver can read aloud if the child feels up to it.
  2. A tiny get-well message: Have the elf hold a small paper note or a simple drawing. Keep the paper clear of equipment and discard it as staff direct.
  3. A pretend rounds visit: Set the elf at an approved spot with a hand-drawn “hello” card. Keep the joke gentle and avoid imitating medical procedures or touching clinical items.
  4. A window watcher: If staff approve the location, sit the elf where the child can see it while looking out the window. Do not attach it to glass, blinds, or fixtures unless the ward says that is allowed.
  5. A quiet story scene: Put the elf beside a permitted book or tell the child a short, silly story about what it got up to overnight. No props are needed.
  6. A choice made from bed: Offer two harmless scene ideas—such as “reading a book” or “holding a drawing”—and let the child choose. A caregiver can do the setup.
  7. A photo from home: If bringing objects is not suitable, show a photo of an elf scene made at home. Share or take photos only in line with hospital privacy rules; do not include other patients or staff without permission.
  8. A sibling-friendly scene: Invite a sibling to suggest a simple idea or draw a picture for the elf, if the ward permits the materials and the child wants to join in.
  9. A resting-day elf: Let the elf “nap” beside a permitted card or book. A child who is tired can simply look, listen, or skip the activity.
  10. A familiar favorite: Have the elf pose beside a drawing or message connected to something the child enjoys. Keep the scene personal rather than assuming every child wants a Christmas activity.
  11. A no-touch hide-and-spot: With staff approval, place the elf somewhere visible from bed and let the child spot it. Do not hide it near supplies or in areas staff need to access.
  12. A one-minute mischief scene: Use one approved paper prop for a small, silly moment—such as the elf “getting tangled” in a drawing. Avoid loose, small, food, or shared items unless the care team explicitly approves them.

Choose an idea that fits the child and the room

There is no need to make the scene elaborate. A hospital stay can change from day to day, so match the activity to the child’s energy, age, mobility, and interest. CHEO describes Child Life support that can include play, seasonal crafts, virtual Santa visits, and decorating patient units; Stony Brook Children’s lists age-appropriate toys, games, crafts, movies, and music. Those programs reflect the importance of tailoring activities—not a guarantee that every child or unit can take part in the same way.

The official Elf on the Shelf store lists scene-prop products, including a kit described by the seller as containing more than 70 interchangeable props, backdrops, and accessories, 24 idea cards, and a recommended age of 3 and up. These are changeable seller details, not a hospital recommendation. Ask the ward before bringing any kit or individual item; permission comes first.

Hospital activity programs also show why checking matters: University of Maryland Golisano Children’s Hospital identifies prepackaged craft kits as useful community projects, while the Association of Child Life Professionals’ bulletin describes screening holiday donations for infection-control concerns and choking hazards. A product being sold for play does not establish that it is appropriate for a particular patient or ward.

What an elf can—and cannot—promise

Hospital Child Life teams use play and seasonal activities to support children and families, but that does not establish a clinical effect for an Elf on the Shelf scene. Treat it as an optional moment of fun, not a treatment or a way to promise a better recovery. As CHEO Child Life Specialist Lisa O’Kane puts it, “Being in the hospital during the holidays can be tough for kids and families.” A small, flexible ritual may suit some families; a quiet day or a different tradition may suit others.
