How does 28 Years Later end?
After his mother Isla dies, Spike briefly returns to Holy Island. He leaves newborn Isla with Jamie and a note explaining that he intends to return to the mainland. The film then jumps forward 28 days: Spike is nearly overwhelmed by infected when Jimmy Crystal and a group of followers arrive and take him in.

The rescue is also a reveal. Jimmy Crystal is the adult version of the boy Jimmy shown at the start of the film, who escaped when infected killed his family. Who is Jimmy Crystal, and what does the ending tell us about him?
The first film establishes Jimmy’s identity and leadership, but its final scene does not fully explain the Jimmies’ beliefs, how their group operates, or what Jimmy wants from Spike. The opening-to-ending connection invites viewers to consider how Jimmy’s childhood experience shaped him; it does not, by itself, settle his motives or psychology.

The sequel’s premise confirms that Spike’s encounter with Jimmy is the handoff. Sony’s official materials say 28 Years Later: The Bone Temple, directed by Nia DaCosta and written by Alex Garland, continues directly after the first film. Its description places Spike’s encounter with Jimmy Crystal at the center of a nightmare, while Dr. Kelson makes a discovery that could change the world. Sony’s official film page and FAQ also describe Jimmy’s followers as a dangerous faction under his leadership.
