It supports one possible on-set accident and points to a reported injury on another film, but neither establishes a complete, well-sourced ranking. It also matters what “fail” means: an accident during filming is not the same thing as spotting a visible double in the finished movie.
Why a verified list of 13 isn’t possible here
The incidents surfaced in available reporting do not match a confirmed, intended set of 13. A roundup that fills the gap with unrelated accidents, dubious claims, or moments when a double is merely visible would suggest a level of verification the evidence does not support. For a documentary-minded account, each entry needs its own reliable source and a clear description of what happened.

There are at least two different subjects hidden in the phrase “stunt double fails”:
- Stunt accidents: someone is injured or a planned effect goes wrong during production. The person involved may be a stunt performer, another crew member, or an actor; those roles should not be conflated.
- Visible doubles: a finished shot makes it possible to notice that a performer is not the star. That is a continuity or filmmaking observation, not evidence of an accident.
Without that distinction, a list can turn production injuries into entertainment or present an ordinary filmmaking choice as a botched stunt.

Two reported incidents—and what the sources establish
The Wizard of Oz: an injury during a broomstick effect
TIME reports that Betty Danko, the Wicked Witch’s stunt double, injured her left leg when the broomstick effect exploded during production of The Wizard of Oz. This is a possible entry in an accident-focused article, but the report alone does not establish that it belongs to any particular list of 13. TIME’s account of The Wizard of Oz.
Back to the Future Part II: don’t turn a reported injury into a death claim
A CinemaBlend report surfaced describing a serious injury to a stunt double during a window sequence in Back to the Future Part II. The available account does not settle every detail, so it should not be treated as fully verified on its own. Crucially, the rumor that an actor died on set was debunked; the reported injury is not evidence of a death. CinemaBlend’s account of the Back to the Future Part II set rumor.

What a responsible roundup would need
Before calling any collection “the 13 biggest,” an editor would need to verify every case and define what “biggest” means. Useful checks include:
- What happened, and whether the event occurred during production or is only visible in the finished film.
- Who was involved: a stunt double, another stunt performer, an actor, or a different crew member.
- The film and production date, the severity of the incident, and the verified outcome.
- Whether the account comes from a primary, contemporaneous, or otherwise high-quality source—and whether a claim has since been corrected or debunked.
Without that evidence, incidents should not be ranked by severity or presented as a definitive set. A short, carefully qualified account is more informative than padding a list to reach a promised number.
