- Izzie Stevens: an on-and-off romance that leads to marriage and, later, a reunion.
- Rebecca Pope: an earlier, complicated relationship.
- Lexie Grey: an earlier relationship after Alex and Izzie’s romance fractures.
- Jo Wilson: a friendship that develops into a central romance and marriage.
This is a guide to Alex’s major romantic storylines, not a complete list of every hookup or brief encounter. A comprehensive inventory of casual involvements is not established by the cited retrospectives.
Alex and Izzie: an on-and-off romance and reunion
Alex and Izzie’s relationship is marked by breaks and reconciliations before they marry. TV Guide’s retrospective describes the relationship as on-and-off and explains that the season 16 departure story reveals Alex left to be with Izzie and their children. Shondaland also includes their marriage in its guide to the show’s major romances.
#1 Best Overall
Alex and Jo: friends who become partners
Alex and Jo’s relationship grows out of friendship. ABC’s official character feature describes them as friends before they become the couple known as “Jolex.” They marry before Alex leaves, making Jo a central partner in his story even though his departure ultimately takes him back to Izzie.
Rebecca and Lexie: earlier, complicated relationships
Rebecca Pope is an earlier, complicated involvement rather than a lasting endgame partnership. The available retrospective does not establish a precise duration, so it is more accurate to describe the relationship without assigning it one.

Alex also dates Lexie Grey after his relationship with Izzie fractures. This is an earlier romance, distinct from the marriages that define his relationships with Izzie and Jo.
