Some ghouls remain lucid and live among other people; others become feral monsters. Their zombie-like appearance is familiar shorthand, but it does not fully describe what they are in the story.
What is a ghoul in Fallout?
Bethesda’s Fallout 76 guide, published March 4, 2025, defines a ghoul as “a person who has been exposed to an inordinate amount of radiation.” Bethesda’s franchise overview describes ghouls as “humans twisted by radiation (and chems).” Together, those descriptions establish the central idea: ghouls began as people and were mutated in the irradiated world of Fallout.
They can resemble zombies, but the word can mislead if it suggests that all ghouls are reanimated corpses or mindless attackers. Fallout includes lucid ghouls with their own lives and stories, as well as feral ones that behave as monsters. “Zombie-like” describes a look or behavior, not the whole category.
Are all Fallout ghouls feral?
No. Bethesda describes ghouls across a range, from wise survivors to terrifying feral monsters. Feral ghouls are one part of the franchise’s portrayal; being a ghoul does not automatically mean losing one’s mind.

The official sources cited here do not give a universal explanation for why or when a ghoul becomes feral. They do not establish that every ghoul inevitably turns feral, identify radiation as the sole cause, or specify a treatment that prevents it. The Feral meter in Fallout 76 is a game mechanic, not a franchise-wide explanation of ghoul biology.
How does becoming a ghoul work in Fallout 76?
Fallout 76 makes ghoul status a playable character system. Bethesda’s March 2025 guide says a player can become a ghoul by completing the “Leap of Faith” quest, which requires level 50. The rules below describe that game system, not a universal set of traits for every ghoul in every Fallout story.

- Food and drink: The player ghoul does not need to eat or drink.
- Radiation: Radiation heals the player ghoul rather than harming them in the usual way.
- Feral meter: It replaces the Hunger and Thirst meters. As it falls, the character becomes more feral: aiming and Charisma decrease, while melee attacks become stronger.
- Glow: It rises with radiation and can add health and activate certain perks. Bethesda Support says Glow scales with accumulated radiation, increases maximum health, and heals damage; foods, drinks, and irradiated environments can contribute to it. These are gameplay rules, not real-world effects of radiation.

Bethesda says the television series is set in the same universe as Bethesda Game Studios’ role-playing games, and its franchise overview discusses ghouls in both games and the show. The shared setting does not mean every story or ghoul-specific detail works identically across each medium.
