Her pale blue eyes suggest a change connected to spice, but the episode does not yet explain what she has become or what her return means. Spoilers below.
Does Lila wake up?
Yes. Tula has secretly kept Lila in the Anirul machine, hoping spice can bring her back after the Agony. Near the episode’s end, Tula experiences a vision. When she comes out of it, Lila has awakened. The episode and recaps present Tula’s use of the machine and spice as the apparent explanation, without fully spelling out how the process works. Parade’s episode breakdown describes Tula trying to rouse Lila as she wakes; The Cinemaholic’s recap also connects her return to the machine and spice.
What do Lila’s blue eyes mean?
Lila’s eyes look pale blue after she wakes, a visual change associated with spice exposure in the episode recaps. It signals that something has happened to her, but “Twice Born” does not settle whether she is the figure described in the prophecy, what memories or consciousness she carries, or whether she poses a danger. Those possibilities remain open rather than confirmed. Parade and The episode’s prophecy and nightmare details describe the clues without resolving their consequences.
Why is Tula frightened?
Several acolytes on Wallach IX experience the same nightmare imagery: sand, an association with a sandworm, and glowing blue eyes. Tula also has a vision as she confronts the violence and secrets of her past. Lila’s awakening comes amid these events, making it understandable that Tula sees it as connected to the threat and prophecy. The episode implies a connection; it does not prove that Lila can stop Desmond or determine what she will do next. Parade’s episode breakdown discusses the shared nightmare and Tula’s vision.

What else happens at the end of episode 4?
Desmond seizes control at the Landsraad
Harrow moves toward an inquiry into Pruwet Richese’s death, but Desmond takes over the meeting. He admits killing Pruwet and calls it an execution for possessing an illegal thinking machine. He then kills the rebels, derailing Valya’s planned political maneuver. Parade’s breakdown covers the Landsraad confrontation.
Theodosia reveals her ability
In the final scene, Theodosia changes her appearance and takes the form of Valya’s late brother, Griffin. The scene establishes that she can change her shape, but not the full limits of that ability or how Valya plans to use it. The episode recap describes the reveal.
