Maki Zen’in and Toji Fushiguro are first cousins through the Zen’in family. Toji was born Toji Zen’in; the Fushiguro surname can make that connection less obvious.
How are Maki and Toji related?
A searchable English translation of the official Jujutsu Kaisen fanbook attributes the family-line explanation to creator Gege Akutami: Toji is the son of Naobito Zen’in’s older brother, while Maki and Mai are the daughters of Naobito’s younger brother. Their fathers are brothers, making Toji and Maki first cousins. The translation also says they did not know one another.

The specific lineage passage is available in an English translation hosted on Scribd, rather than on the official publisher page. Shueisha’s official fanbook page confirms the book’s existence, but does not itself establish the details in that translated passage. Treat the relationship as the fanbook’s reported answer, not as a verified verbatim quotation; consult the licensed edition for exact wording.
Why Toji’s surname can hide the connection
Toji is commonly known as Toji Fushiguro, but the fanbook translation gives his birth name as Toji Zen’in. His later surname does not change his Zen’in family lineage, which is the basis for his cousin relationship with Maki.

How their relationship differs from their story parallels
Maki and Toji are often discussed together in connection with their physical abilities and their place in the Zen’in family story. Those parallels are separate from the evidence of kinship: the reported family tree establishes that they are cousins, while similarities between their abilities or narrative roles do not prove that relationship.
For story context, VIZ lists Chapter 149, relevant to the Maki-and-Toji comparison. The listing identifies the chapter but does not, on its own, verify particular dialogue or page-specific claims.

Where to check the source material
The relationship detail comes from the fanbook’s character Q&A as rendered in the searchable English translation hosted on Scribd. VIZ describes Jujutsu Kaisen: The Official Character Guide as containing character profiles and creator material, making it a related reference.
