¿Cuál fue la novedad?
La FDA aprobó Opzelura, una crema de ruxolitinib, para el vitiligo no segmentario en pacientes de 12 años o más. La FDA la describió como el primer tratamiento farmacológico aprobado para abordar la repigmentación en personas con vitiligo. La aprobación se refiere específicamente al vitiligo no segmentario, la forma más común, que causa parches de pérdida de pigmento y puede afectar zonas como la cara, el cuello, el cuero cabelludo, las manos y los brazos.

Cómo se usa Opzelura según la información de prescripción
La etiqueta estadounidense indica aplicar la crema dos veces al día en las zonas afectadas, que en conjunto no deben superar el 10 % de la superficie corporal. La pauta debe seguirse de acuerdo con la información de prescripción vigente y las indicaciones del profesional que atiende al paciente. La etiqueta señala que puede ser necesario continuar más de 24 semanas para observar una respuesta satisfactoria; si para entonces la repigmentación no es significativa, el profesional debe reevaluar el tratamiento.

Qué mostró el ensayo, y qué no permite concluir
En los ensayos que respaldaron la aprobación, el 30 % de quienes recibieron Opzelura y el 10 % de quienes recibieron placebo alcanzaron al menos un 75 % de mejoría en el índice de área de vitiligo facial (F-VASI) después de 24 semanas. Es un resultado de ensayos clínicos para una medida facial concreta, informado por la FDA en 2022; no es una predicción individual ni una garantía de que toda persona tratada vaya a lograr esa mejoría.

La FDA enumera entre las reacciones adversas frecuentes acné en el lugar de aplicación, picazón y enrojecimiento, resfriado común, dolor de cabeza, infección urinaria y fiebre. La información de prescripción estadounidense indica que no se recomienda combinar Opzelura con medicamentos biológicos terapéuticos, otros inhibidores de JAK ni inmunosupresores potentes como azatioprina o ciclosporina.
