P. Venugopal led India’s first successful heart transplant at AIIMS in New Delhi in 1994. In an excerpt from Heartfelt: A Cardiac Surgeon’s Pioneering Journey, written with Priya Sarkar, he recalls the night the donor heart was brought into the operating theatre and the operation that followed. A peer-reviewed historical review also places the milestone in the context of India’s recognition of brain-stem death.

Who performed India’s first successful heart transplant?

Cardiac surgeon P. Venugopal led the operation at the All India Institute of Medical Sciences (AIIMS) in New Delhi in 1994. The peer-reviewed review “Heart transplantation in India—looking back as we celebrate 25 years of the transplant law” identifies his procedure as India’s first successful heart transplant.

The account in Heartfelt: A Cardiac Surgeon’s Pioneering Journey, by Venugopal and Priya Sarkar and published by HarperCollins India, is a first-person memoir rather than an independent clinical record. Scroll.in published an excerpt on July 13, 2023, under the title “From the memoir: Surgeon who performed India’s first heart transplant in 1994 tells the story.”

What happened during the 1994 operation?

Venugopal’s memoir begins around 11 p.m. on August 2, 1994, as he waited for a suitable donor. It identifies the recipient as Devi Ram, a 40-year-old lathe worker with AB-positive blood. The donor was a woman in her mid-thirties who, according to the memoir, had been certified brain dead and was eligible to donate.

The donor heart was removed in an adjacent operating theatre and brought to the recipient’s theatre. Venugopal recalls that the heart was implanted and working on bypass in 59 minutes, after which it began beating on its own. As the memoir puts it: “It took all of fifty-nine minutes to have the heart in and working on the bypass – and then it started beating on its own!” That duration and sequence are details from his recollection, not a separately verified measure of the operation.

Why did brain-stem death matter?

The timing of the transplant coincided with a major legal change. The peer-reviewed review states that Parliament passed the Transplantation of Human Organs Act on July 8, 1994, recognizing brain-stem death and making deceased-donor transplantation possible; rules followed in 1995. In the memoir’s account, the donor’s certification and eligibility are central to how the heart became available for transplant.

The operation took place the following month. The review’s chronology helps explain why the transplant is remembered not only as a surgical milestone but also alongside the legal recognition that enabled deceased-donor organ donation.

How was the 1994 operation different from the earlier attempt?

The memoir describes an earlier human-to-human transplant attempt by Dr P.K. Sen at King Edward Memorial Hospital in 1968. The patient died within hours. That attempt is distinct from the first successful heart transplant, which Venugopal led at AIIMS in 1994. The historical review likewise distinguishes earlier attempts from the successful milestone.

What followed the transplant?

Venugopal recounts the public announcement and the attention that followed, with newspaper headlines appearing on August 4, 1994. His memoir describes a mixture of acclaim and skepticism around the achievement. These reactions are part of the excerpt’s account of the event’s public reception.

How did heart transplantation spread across India?

The review’s table of first successful transplants by state or union territory traces later regional milestones. The dates below refer to those first successful procedures as listed in that review; they are not a complete account of subsequent transplant activity.

Location Surgeon named in the review Year
New Delhi P. Venugopal 1994
Tamil Nadu K. M. Cherian 1995
Kerala Jose Chacko Periappuram 2003
Telangana Alla Gopala Krishna Gokhale 2004

For the operation’s first-person story, the Scroll.in excerpt offers Venugopal’s recollection; the peer-reviewed review provides historical and legal context. Together, they place the AIIMS transplant within both a surgical history and a changing framework for deceased donation.
