Her paternal grandparents were John Barrymore and Dolores Costello; her great-grandparents were Maurice Barrymore and Georgiana Drew.

How is Drew Barrymore related to Ethel and Lionel Barrymore?
Ethel and Lionel Barrymore were siblings of Drew’s grandfather John Barrymore. That makes Ethel Drew’s great-aunt and Lionel her great-uncle. They belong to the same well-known Barrymore family, but they are not Drew’s grandparents.
This distinction matters when tracing the tree: John and Dolores are direct ancestors, while Ethel and Lionel are collateral relatives—family members connected through a sibling rather than a direct parent-to-child line. Biography.com Los Angeles Times

Where the Drew and Costello branches come in
The Drew theatrical line
Georgiana Drew’s parents were performers and theater figures John Drew and Louisa Lane Drew. Her marriage to Maurice Barrymore joined the Drew and Barrymore branches. Accounts of the family trace its theatrical history back well before Hollywood: the Los Angeles Times reported in 2009 that family members performed before the American Revolutionary War, while Biography.com traces the Drew line to documented performers in eighteenth-century Britain. These accounts describe the limits of the history they document, not proof that the family’s performance tradition began at a particular date.
The Costello connection
Dolores Costello’s father was Maurice Costello, an early screen actor, adding another performer’s family to Drew’s ancestry. The Barrymore family’s work spans stage, silent film, sound film and television; the Drew branch is associated especially with theater, while the Costello connection reaches into early cinema. What Drew Barrymore has said about her acting ancestors
In a 2009 CBS interview with Anderson Cooper, Barrymore reflected on the family’s theatrical roots: “Going back to Georgiana Drew and John Drew, and my great-grandfather Maurice Barrymore, and it was such a sort of circus of odd, interesting people that loved acting.” CBS News
Her description captures the family’s long performance history, but the tree itself is a record of relationships—not evidence that talent is inherited or that family background determined her career. Drew has two daughters, Olive and Frankie; that fact alone does not establish them as part of the family’s historic acting profession. Biography.com
