The Substance is a 2024 horror film about a woman offered a younger, more beautiful version of herself—and the cost of trying to live up to that promise. Demi Moore leads as Elisabeth Sparkle, with Margaret Qualley as Sue, her younger counterpart. Coralie Fargeat’s precise, richly colored filmmaking and escalating body horror make the film a striking watch; whether its critique of ageism escapes the spectacle it creates is a more unsettled question.
What is The Substance about?
Elisabeth Sparkle is a former star who hosts a fitness television show. After the studio fires her, she turns to a mysterious treatment that temporarily transforms her into Sue, a younger version of herself. The two bodies must alternate one week at a time, a strict arrangement that becomes harder to maintain as Sue gains a new celebrity life.
The premise turns a familiar promise of self-improvement into a science-fiction nightmare. The treatment is marketed as a way to become “you, but better in every way,” but the film’s central conflict is not simply about looking young: it is about who gets to occupy the spotlight, and what happens when one version of a person is valued more than the other. The official synopsis and Cannes description outline the setup (Festival de Cannes; Variance Films).
Does Demi Moore lead the film?
Yes. Moore plays Elisabeth, and Qualley plays Sue. Dennis Quaid also appears. The film’s two central performances are bound together by its premise, but Elisabeth’s experience gives the story its emotional anchor: the treatment’s promise is inseparable from the fear of becoming invisible.

Associated Press critic Krysta Fauria describes Moore’s performance as powerfully vulnerable, while BFI’s Kelli Weston also praises her work. Those responses point to the human tension beneath the film’s deliberate excess: Elisabeth is not just confronting a transformation, but the conditions that make it seem desirable.
How does the film look and feel?
Fargeat gives the film a controlled, polished visual language before pushing it into increasingly grotesque territory. Weston highlights its precision, symmetry and saturated colors: vivid reds, bright yellows and deep Gothic blues. That elegant framing sits in productive tension with the bodies and images on screen, making the visual beauty part of the film’s unease rather than a neutral backdrop.

Fauria describes a setup that begins closer to conventional science fiction before erupting into absurdist violence and a monster-film third act. The shift is a defining feature: gore becomes both shock and satire, and the film’s excess can feel cathartic, comic or exhausting depending on a viewer’s tolerance for it. AP finds the escalation entertaining; BFI’s reservations focus more on the film’s argument and references than on its craft.
Does its satire of beauty culture work?
The film frames its transformation as a response to the demands of youth, celebrity and beauty. Its target is not only a fictional studio’s standards, but a culture in which a woman’s value can seem tied to how young she looks and how much attention she attracts. Associated Press reads the film as commentary on ageism and sexism in Hollywood.

That critique is also where the film becomes most debatable. The body horror makes the violence behind impossible standards impossible to ignore, but it also turns the older female body into an object of spectacle. BFI questions whether the film’s “monstering” of that body gives it a coherent politics of aging. The contradiction is worth keeping in view: the film may expose the gaze while relying on it for some of its most confrontational images. Its polished beauty and grotesquerie sharpen the question without resolving it.
How graphic is The Substance?
Expect intense body horror, bloody violence and graphic imagery, with nudity and other potentially distressing material. The UK BBFC classifies the film as drama/horror and lists “strong bloody violence, gory images.” Its detailed advice also mentions graphic gore and injury, full nudity, sexist and ageist remarks, drug injection, self-harm, and flashing or flickering images that may affect photosensitive viewers. Check the BBFC’s UK content advice if specific content could affect your decision to watch.

In the United States, Associated Press reported an R rating for strong bloody violent content, gore, graphic nudity and language. That is a US rating, not a substitute for the UK classification or local guidance elsewhere.

Plan for about 140 minutes. The BBFC’s UK classification records list 141 minutes, with its cinema record giving 141 minutes 7 seconds and its home-entertainment record 141 minutes 4 seconds; AP reported 140 minutes in its 2024 review. Working Title Films lists 20 September 2024 as the UK release date, which is territory-specific rather than a universal release date (Working Title Films).

MUBI has an official film page, and the BBFC names MUBI as distributor in UK classification records. That does not establish current streaming availability in every country: check the local catalog for the service and territory where you plan to watch (MUBI). The BBFC also records a home-entertainment classification, but that alone does not confirm the contents or current stock of any particular physical edition.
Is The Substance worth watching?
It is a strong choice for viewers who want boldly designed horror, a committed central performance and body horror that grows more extreme as the story unfolds. Its images are carefully composed even when they become repellent, and the escalating excess gives the film a vivid, sometimes darkly comic momentum.
It is less satisfying if you want a subtle or fully coherent argument about aging. The movie’s critique of beauty culture is forceful, but its visual fascination with transformation and bodily spectacle complicates that critique. That tension is not a small footnote to the experience; it is one of the main reasons the film invites disagreement.
