Episode 7 reveals that Lois Tryon—not her husband Marshall—is the person in a coma. The murder mystery and relationships shown in the first six episodes have been presented as Lois’s dream. She wakes after her fight with Megan, but the shift in reality is not framed as the show’s final layer: Ryan Murphy and FX chairman John Landgraf told TheWrap that the reveal was built into the series and that more levels of meaning remain.
What happens at the end of Grotesquerie Episode 7?
The first six episodes play as a murder story led by detective Lois and journalist nun Sister Megan. Episode 7 changes how viewers should understand that story: Lois is in a coma, and the events and relationships that came before have been presented as her dream. The episode returns to Lois’s fight with Megan; after Lois gets the gun, the narrative shifts to Lois waking in the hospital, as TVLine’s recap describes.
That is the specific reveal. It does not, by itself, settle what every earlier image means or how the waking-world story will unfold.
Who is who in the waking world?
The switch keeps familiar faces but rearranges their roles. TVLine summarizes the waking-world connections this way:

- Compact & Travel-Friendly: The ultra-compact design allows the adapter to stay plugged into your device without blocking adjacent ports or adding bulk, reducing wear and tear on your original USB ports.
- 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.
- Lois: the patient in a coma who wakes at the episode’s end.
- Marshall: Lois’s husband.
- Megan: a detective, rather than the journalist nun seen in the dream narrative.
- Charlie: a doctor.
- Ed: Lois’s son-in-law.
These changed roles make the waking world distinct from Lois’s dream, but they are not a complete explanation of what the characters’ relationships or the ongoing mysteries will become.
Was the coma twist planned?
Murphy told TheWrap that the twist had been part of the show’s design from the start. “I’ve never done anything that worked out in advance, that meticulously designed. It was so fun to do,” he said. His comment supports reading the coma reveal as an intended structural turn, rather than a last-minute explanation supplied after the fact.

The episode also contains visual details Murphy connected to the conceit. He pointed to the burning curtain as an echo of the curtains around Lois’s hospital bed, and to Nurse Redd eating cherries as a subconscious reference to fear of blood clots. Those are creator-identified clues, not a key that automatically explains every strange detail in the first six episodes.
Why Murphy says the reveal is only the first layer
In the same interview, Landgraf likened the series to a Russian nesting doll with “multiple different levels of meaning past the first reveal.” He also said, “We’re on the edge of a cliff, and we’re going to jump.” The remarks signal that the coma explanation is a new point of entry into the story, not a promise that a particular number of twists or a specific resolution will follow.

Murphy described Lois as needing to confront the people in her life after waking. Micaela Diamond, who plays Megan, likewise told TheWrap that later episodes continue to explore the boundary between dream and waking reality. Together, those comments leave the emotional weight of the dream intact while keeping the new reality’s development open.
