Season One does not reveal the real-world Grotesquerie killer. Episode Seven appears to expose Sister Megan Duval as the murderer, but that accusation—and the crimes investigated in the first six episodes—takes place in Detective Lois Tryon’s coma dream. The finale returns to murder imagery in Lois’s apparent waking reality, yet ends before naming the culprit. Megan is the killer in Lois’s dream narrative; the actual killer remains unidentified at the Season One endpoint.
Was Sister Megan really the killer?
Only within the story Lois dreams while comatose. In the first six episodes, Lois investigates ritualized killings with Sister Megan, a journalist nun drawn to true crime and the occult. Lois eventually accuses Megan, and their investigation culminates in a violent confrontation. Episode Seven changes the context: the case Lois has been pursuing was part of a vivid coma dream, not a solved account of the real-world murders.
That makes two answers necessary. Inside the dream, Megan is presented as the culprit. In the later story, after Lois wakes, the series does not confirm that Megan—or anyone else—is responsible for the crimes.
What does Episode Seven reveal?
The episode reveals that Lois has been comatose and reframes the preceding crime story as a dream. Creator Ryan Murphy said the coma premise was built into the story from the beginning. He and FX chairman John Landgraf described the turn as an intermediate mystery rather than a simple “it was all a dream” reset; Niecy Nash-Betts also said Lois is awake after Episode Seven.

After the reveal, the series shifts to people and relationships that Lois’s dream had transformed. The coma explains why the first six episodes play by dreamlike rules, but it does not explain away everything that follows: the later story brings back murder imagery in what the show presents as Lois’s waking reality.
Which clues point to Lois’s coma?
Murphy identified several details as intentional signals connected to Lois’s hospital condition. They help explain the Episode Seven reveal; they are not a trail that identifies the real-world murderer.

The burning curtain
Murphy said the opening image of a curtain on fire nods to the curtains around Lois’s hospital bed. The image gains a second meaning once viewers know where Lois is.
Nurse Redd’s cherries
Murphy said the cherries represent Lois’s subconscious fear of blood clots. The bodily anxiety sits inside the story’s surreal imagery, rather than serving as evidence against a suspect.

The white light
TheWrap reported Murphy’s explanation that a moment when Lois is struck by white light signals her ascending to a “higher plane.” That is his account of the scene’s design, not medical evidence about coma or recovery.
Eddie’s first appearance
Murphy said Lois’s first meeting with Eddie was staged to feel like a dream and likened him to a literal guardian angel. The encounter’s unusual, unreal quality fits the coma-story framework.

Dreamlike staging across the first six episodes
Murphy told Vanity Fair that the opening episodes contain “coma Easter eggs” and that the puzzle is built around a dual reality. FX’s Behind the Crimes – Crafting The Plot Twist feature likewise presents the cast and crew’s discussion as a trail of clues leading to Episode Seven. Together, these details support the coma reveal, not a verdict about who commits the later murders.

Lois and Megan discover another grotesque tableau modeled on Leonardo da Vinci’s The Last Supper, with Father Charlie cast as the murdered Christ figure. Its echoes of the dream’s crimes make clear that the later story has not closed the season’s whodunit.

Lois says she knows who the killer is, but the finale ends before she gives a name. Nash-Betts described the identity as “a huge unanswered question” in finale coverage by TVLine. The season therefore leaves the real-world killer unidentified.
What the clues do—and do not—prove
- They establish the story’s first layer: Megan is accused as the killer in Lois’s coma narrative.
- They explain the twist: the fire, cherries, white light, Eddie encounter and dreamlike staging support the coma reveal.
- They do not name the waking-world killer: the finale withholds the name Lois says she knows.
- They do not confirm a suspect theory: theories involving Lois’s family, workplace or medical circle remain speculation. Megan, Charlie, Nurse Redd and Lois are not established as the real-world culprit.
Murphy told TheWrap that the twist had been meticulously designed in advance, while his broader question for the series, as he put it to Vanity Fair, is whether what people are experiencing is really happening. That thematic uncertainty is part of the show’s structure; it should not be mistaken for evidence naming a murderer.
