Link asks for cautery as surgery resumes, and the finale cuts to the blast. It does not confirm who is killed or injured. Meredith is outside the hospital and has decided to stay at Grey Sloan rather than sell her shares.
What happens in the season 21 finale, “How Do I Live”?
Jenna Gatlin takes Amelia and Simone hostage in an operating room, desperate to save her nine-year-old daughter, Dylan. Lucas brings Dylan into the OR, and Amelia operates under threat and without the full preparation she would normally have. Monica recognizes the danger, alerts Webber, and the hospital begins evacuating while staff try to contain the situation.
Meredith is at Grey Sloan intending to sell her shares to Catherine Fox to help fund her Alzheimer’s research. After reviewing Dylan’s case, Meredith recognizes that Dylan needs fresh frozen plasma and brings it to the OR. Amelia and Lucas stop the bleeding in Dylan’s brain. Jenna says the acetylene tank is empty, the police enter, and she is arrested. The apparent hostage crisis seems to be over.
Elsewhere, Teddy and Winston operate on Nora Young, whose infected graft requires an experimental aortic bypass. The procedure succeeds. Teddy takes the outcome as a reason to reconsider her life with Owen and says she wants to choose herself.

What causes the Grey Sloan explosion?
The tank was not empty. After Jenna is arrested, Dylan’s father tells Amelia there were no empty tanks on his truck. Jenna had opened the valve and released acetylene into the OR area. When surgeries resume, Link asks for the cautery instrument during an operation. The episode then cuts to an explosion seen from outside the hospital as Meredith leaves.
The sequence establishes the cause of the blast, but not the specific injuries or deaths. Showrunner Meg Marinis told TheWrap on May 15, 2025: “There will definitely be consequences to this explosion, just to who and how will be determined when everyone tunes in [this fall.]” She also said, “We don’t know, and when I say, ‘We don’t know.’ I mean y’all don’t know.” Those remarks describe the deliberate cliffhanger, not a confirmed list of victims.

Who is in danger—and who died?
The finale does not confirm that any named character dies, nor does it confirm that anyone shown near the operating floor is safe. Bailey, Monica, Jules, and Link are identified on the OR floor when the explosion happens; Lucas may be moving through the crowded area to warn Webber. Meredith is shown outside in the parking lot.
ABC’s season 22 episode guide says the premiere begins in the aftermath, with staff scrambling to save patients and themselves. That confirms the explosion carries into the next season, but the brief official synopsis does not settle each character’s fate.

What happens to Meredith?
Meredith changes her mind about selling her shares to Catherine. She returns to Grey Sloan as a general surgeon for the summer, keeping her work at the hospital connected to her research plans. Her decision is separate from the explosion: she is outside when the blast occurs.
What do the other season-ending cliffhangers mean?
- Teddy and Owen: Teddy says she wants to choose herself, putting her marriage to Owen in question.
- Simone and Lucas: Lucas declares his love, but Simone must tell him she slept with a new intern after their breakup.
- Jules and Winston: Jules confronts Winston for letting his crush interfere with her education and says the feelings were not mutual.
- Jo and Link: Jo tells Link they are expecting twin girls shortly before he returns to surgery. The explosion interrupts the moment of celebration.
- Ben’s residency: Teddy will not recommend Ben to continue in Grey Sloan’s surgical program. After Ben saves a patient in the ICU, Bailey assures him they will find another residency program.
What the season 22 setup confirms
The official ABC listing frames the next season’s opening around the immediate aftermath of the explosion: staff are trying to save patients and themselves. It does not provide character-by-character outcomes, so the season 21 ending remains unresolved on that point. U.S. viewers can find Grey’s Anatomy through ABC’s show page, which identifies Hulu as a streaming option.
