The direct answer: Isla dies from terminal cancer with Dr. Ian Kelson’s help; Erik is killed by the Alpha infected Samson; Jamie and Spike survive; and Spike leaves the newborn Isla on Lindisfarne before returning to the mainland. The closing rescue by Sir Jimmy Crystal’s gang is deliberately ominous: it introduces organized human evil just after Spike has learned compassion and acceptance from Kelson.

Spike brings the newborn back to Lindisfarne, names her Isla after his mother and leaves her with the island community. He does not stay. Jamie remains alive on the island, but the rising tide prevents him from following Spike across the causeway. Spike returns to the mainland, where he is later rescued by a violent, highly theatrical gang led by Sir Jimmy Crystal.

The final twist is that Jimmy is strongly implied to be the child from the opening scene—the boy watching Teletubbies as the original outbreak begins. The ending changes the film’s threat from infected violence to organized human evil. Jimmy’s group saves Spike, but the rescue looks more like recruitment than salvation.

What happens to Isla?

Spike crosses from Lindisfarne to mainland Britain with Isla because her condition is worsening. She is suffering pain, confusion and deteriorating consciousness, and Spike believes that Dr. Ian Kelson may be able to cure her. His journey begins as an attempt to save his mother, but the film gradually forces him to understand that not every death can be prevented.

Kelson examines Isla at his sanctuary and discovers that she has terminal cancer, including cancer that has spread to her brain. She is not infected, and there is no realistic treatment that can restore her health. With Isla’s consent, Kelson helps her die on her own terms rather than allowing her final days to become prolonged suffering.

This is the emotional climax of the film. Spike’s apparent mission has failed in its literal sense—he cannot bring his mother home cured—but the film does not present him as having failed Isla. He gets her to someone who understands her condition, stays with her through the farewell and allows her wishes to matter.

Kelson’s philosophy is summarized by the idea of memento mori: remembering that death is unavoidable. His bone temple is not merely a macabre hideout. It is a memorial to the people who died during the outbreak, and it gives death a place within a continuing human community. Isla’s death teaches Spike that accepting an irreversible loss is not the same as giving up on life.

Who dies in 28 Years Later?

After Isla’s death, Spike returns to Lindisfarne carrying the newborn girl. He names her Isla, preserving his mother’s name without pretending that his mother can be brought back. He leaves the baby with the islanders and a note explaining that Kelson is not mad.

The choice completes Spike’s emotional journey. At the start, he is focused on winning his father Jamie’s approval and finding a cure for his mother. By the end, he has accepted that he cannot save Isla, but he can still protect another vulnerable person. The newborn becomes a practical responsibility as well as a symbol of continuity.

The film deliberately places two images beside each other: Kelson’s bone memorial, which honors the dead, and the newborn child, who represents life continuing after catastrophe. The baby does not erase Isla’s death. Instead, Spike carries Isla’s memory forward while making a new choice to care for someone else.

Is the baby immune to the Rage Virus?

The film does not establish that the baby is immune, and it does not establish that she is an asymptomatic carrier. She is born to an infected mother and shows no immediate signs of infection, but that is all the ending confirms.

Several explanations remain possible: the baby could be uninfected at birth, resistant to infection, infected in a way that has not yet become visible, or part of a form of transmission the film has not explained. Treating her as definitively immune goes beyond the evidence in this film.

What is clear thematically is that the baby represents possibility. The infected world has not produced only death and repetition; it may also contain biological change that the survivors do not understand. Her future is one of the film’s unresolved sequel questions.

Why can’t Jamie follow Spike?

Spike returns the baby to the island but then chooses to go back to the mainland. Jamie tries to follow him, yet the tide cuts off the causeway between Lindisfarne and Britain. Jamie is therefore alive at the end, physically separated from his son by the same geography that protected the island community.

Spike’s decision is also a rejection of the life Jamie has tried to impose on him. Their relationship has been damaged by Jamie’s harsh survivalist expectations and by Spike discovering that his father is having an affair. Spike began the story trying to become the son Jamie wanted. He ends it by making an independent moral decision: to keep moving and to help where he can, even though the mainland is dangerous.

His statement that he wants to continue walking until he can no longer see the water gives the ending its bittersweet shape. He is grieving, but he is not simply running away. He is choosing a future outside the narrow identity his father defined for him.

Who is Sir Jimmy Crystal?

Sir Jimmy Crystal is strongly implied to be the same Jimmy seen in the film’s opening sequence. As a child, Jimmy watches Teletubbies while the first outbreak begins. His preacher father gives him an upside-down crucifix and accepts the infected as a form of religious judgment. The adult Jimmy wears the same distinctive cross, and Jack O’Connell plays the character credited as Sir Jimmy Crystal.

The film does not pause for a conventional flashback or a line of dialogue spelling out the connection. Instead, it uses recurring visual and verbal clues: the necklace, the opening-and-ending structure, Jimmy graffiti and the name itself. The implication is clear while still leaving room for the sequel to explore what happened to the child after the opening massacre.

Jimmy is not the original 28 Days Later protagonist Jim. He is a different character: the child from the prologue, now an adult leader whose group has developed its own disturbing mythology.

Why are the Jimmies dressed like Jimmy Savile?

Jimmy’s blond hair, tracksuits, jewelry and celebrity-style performance evoke Jimmy Savile, the British television personality whose public image was fundamentally reinterpreted after the posthumous exposure of his serial sexual abuse.

The reference is not a claim that the fictional Jimmy is literally Savile, nor is the gang meant to be harmless comic relief. The film presents the imagery as disturbing. Jimmy appears to have preserved a child’s memory of an apparently charismatic television celebrity from the early 2000s, before Savile’s crimes were widely known. In the post-apocalyptic world, that cultural memory has been warped into the costume and mythology of an adult predator.

That is why the visual joke becomes increasingly uncomfortable. The gang’s style is a portrait of cultural memory detached from context and morality. A performer once associated with entertainment is transformed into a model for dominance, spectacle and menace. The allusion has also attracted criticism because it invokes a real person whose abuse caused profound harm; it should be discussed as a deliberately provocative reference, not treated as a throwaway gag.

Why does the film suddenly become so stylized and violent?

The central ending is subdued and intimate: a terminal diagnosis, a consensual death, a memorial and a newborn child. Then the film abruptly cuts to Jimmy and his followers performing acrobatic attacks while massacring infected people, with Teletubbies audio playing over the violence.

This tonal rupture is intentional. Danny Boyle has described the ending as a return of evil to an environment that had briefly become compassionate, and has contrasted the first film’s concern with family with the next chapter’s focus on evil. The style is not merely an action spectacle added after the emotional story. It is the point.

  • It completes Spike’s first arc. He has learned that death cannot always be defeated, but he still chooses to protect the baby and continue living.
  • It reverses the moral atmosphere. Kelson represents compassion, ritual and coexistence with death. Jimmy represents charisma, domination, hierarchy and organized cruelty.
  • It changes the nature of the threat. The infected are dangerous, but Jimmy’s group demonstrates that surviving humans can deliberately turn violence into identity and entertainment.
  • It launches the next story. Spike is no longer simply walking through infected territory. He has entered the orbit of a human group that may be more dangerous because it can recruit, organize and manipulate.

Jimmy’s gang rescues Spike after he is attacked by infected people 28 days after leaving the island. But the rescue is ominous. Spike has just lost his mother and separated from his father, leaving him emotionally exposed. Jimmy’s offer of belonging may therefore be a recruitment tactic aimed at a vulnerable teenager, not an act of uncomplicated heroism.

What does the ending say about family?

The film compares four different versions of family:

  • Jamie’s family: survival through toughness and control. Jamie wants to prepare Spike for a brutal world, but his affair and secrecy weaken the authority he claims.
  • Isla’s family: protection grounded in honesty and emotional care. Even while dying, Isla remains the person Spike is trying to honor rather than impress.
  • Kelson’s chosen community: compassion extended beyond conventional relatives. His sanctuary gives the dead dignity and gives the living a humane way to confront loss.
  • Jimmy’s gang: a counterfeit family built around spectacle, hierarchy and shared performance. It offers identity and belonging, but at the cost of moral independence.

Spike moves through these models in order. He begins by trying to earn Jamie’s approval, then chooses Isla’s welfare, learns from Kelson, protects the newborn and finally encounters Jimmy’s seductive alternative. The ending does not show Spike accepting Jimmy’s worldview. It stops at the dangerous moment when Jimmy’s group invites him into it.

What does the ending set up for the sequel?

28 Years Later was designed as the first part of a continuing story. The next film, 28 Years Later: The Bone Temple, expands the Jimmy storyline and follows the consequences of Spike’s encounter with the gang. It was directed by Nia DaCosta and positioned as the next chapter after Boyle’s film.

However, the first film does not answer every question about Jimmy. It does not fully explain his ideology, how his group recruits people, what happened to him between the prologue and the present, whether Spike joins willingly or under pressure, or what the baby’s biological status ultimately is. Those should remain marked as sequel questions rather than treated as already-resolved facts.

Final interpretation: what does the ending mean?

The ending argues that survival is not enough. Spike cannot cure Isla, cannot repair his parents’ relationship and cannot make the mainland safe. What he can do is accept the truth, honor the dead, protect the living and decide what kind of person he will become.

Kelson gives him a language for accepting death. The baby gives him a reason to keep caring. Jamie’s separation shows the cost of leaving his old family behind. Jimmy then appears as the corrupt alternative: a charismatic figure offering belonging through violence and performance.

That is why the final sequence feels both triumphant and frightening. Spike is saved from the infected, but he may have been delivered into a more sophisticated danger. The film ends not with a solution, but with a moral crossroads: whether Spike will carry Isla’s compassion forward or be absorbed into Jimmy’s theatrical, predatory version of family.
