Exodus Tyson was four years old when she was fatally injured in a treadmill-cord accident at a home in the Phoenix area on May 25, 2009. Phoenix police described the incident as a tragic accident and said the initial investigation found nothing suspicious. She died at a hospital the following morning, May 26.
The available contemporary reporting does not establish exactly how the cord became positioned around her neck. Police said she may have slipped or put her head into a loop hanging beneath the treadmill console. Those possibilities should remain qualified: the precise movement was never established in the reports reviewed.
What happened to Exodus Tyson?
According to an account attributed to the Phoenix Police Department and reproduced by contemporary news organizations, the accident happened on Monday, May 25, 2009, at a Phoenix-area home. Exodus was playing on or near a treadmill when a cord or cable attached to the machine became caught around her neck.

The reports do not identify the treadmill’s manufacturer or model. They also do not establish that the belt was running. Early descriptions of the incident included an incorrect radio report of electrocution, but Phoenix police later said that account was wrong. The confirmed reporting describes a cord-related injury, not electrocution.
Police offered more than one possible explanation for how the entanglement occurred, including the possibility that Exodus slipped or placed her head into a loop hanging below the treadmill console. Because no definitive reconstruction is provided in the reliable accounts reviewed, it would be misleading to state one version as fact.
Her brother found her and her mother called 911
Exodus’s mother was cleaning elsewhere in the home and asked her seven-year-old son to check on his sister. He found Exodus in distress and alerted their mother.
Her mother removed the cord, called 911, and began CPR. Phoenix police officers arrived, followed by firefighters and other emergency personnel. Rescuers continued attempts to revive the child before transporting her to a local hospital in extremely critical condition.

This account is important because it reflects the emergency response described by police and contemporary reports: the child was discovered by her brother, her mother immediately sought help and attempted CPR, and professional responders performed emergency resuscitation. It does not establish that anyone could have predicted the accident or that a particular safety device would have prevented it.

How Mike Tyson later spoke about the loss
Tyson discussed Exodus’s death publicly in later interviews, while generally keeping the family’s private circumstances out of the public record.
In an October 2009 appearance on The Oprah Winfrey Show, as reported by the Associated Press, Tyson called Exodus “my angel.” The report said he did not want to know every detail of what had led to her death. It also described his initial rage and his later statement that he no longer felt animosity toward anyone.
Those remarks document Tyson’s public response at that time; they should not be treated as a complete psychological assessment or as proof of a particular therapeutic process.
In a December 2, 2010 CNN interview, Tyson referred to the death as a “freak accident on the treadmill,” confirmed that he had been in Las Vegas when it happened, and called it the worst incident of his life. He also reflected that grief does not simply disappear and that the living must continue. These comments offer Tyson’s own later framing of the loss, rather than an objective clinical description of grief.

What the tragedy does—and does not—show about treadmill safety
A treadmill can present risks to children even when an adult is not actively exercising. The appropriate safety lesson is not that every treadmill has precisely the same hazard, or that one particular device would necessarily have prevented this accident. It is that exercise equipment should be physically inaccessible to young children when it is not being used, and closely supervised when it is in use.
Practical precautions for treadmill owners
- Keep children away from the treadmill. Use a closed room, physical barrier, or other arrangement that prevents unsupervised access.
- Do not leave children alone near the machine. Safety devices reduce risk but do not make equipment completely childproof.
- Manage cords and loops. Follow the owner’s manual for routing and securing cables, and keep loose cords out of a child’s reach.
- Know the emergency-stop control. Adults using the treadmill should know how to stop it quickly and should wear the safety key when the model uses one.
- Wait for the belt to stop completely. Do not step off a moving treadmill, and do not allow children to approach it until it has stopped.
- Inspect the machine. Replace damaged cords or missing safety components and follow the manufacturer’s maintenance instructions.
U.S. Consumer Product Safety Commission childproofing guidance makes the broader point: safety devices work only when correctly installed and used, and no device is completely childproof. The agency has also documented continuing treadmill concerns involving children reaching beneath or being pulled under moving treadmill components. These general safety concerns should not be confused with a definitive reconstruction of the 2009 accident.

- PROTECTS FLOORS FROM HEAVY EQUIPMENT – Goes under a treadmill, exercise bike, elliptical, or rower so the machine sits on the mat instead of grinding into your hardwood, tile, carpet, or laminate. Takes the scuffs, dents, and sweat so your floor doesn't.
- CUTS NOISE AND VIBRATION FOR THE PEOPLE BELOW YOU – The 1/4 inch foam absorbs the thump and hum from a running treadmill or a spinning flywheel before it reaches the floor. Quieter for you, quieter for whoever lives downstairs.
- NON-SLIP SURFACE KEEPS MACHINES PLANTED – Textured top grips the equipment, flat bottom grips the floor. Your bike or treadmill stays where you put it instead of walking across the room mid-workout.
- 60 x 36 INCHES FITS UNDER MOST CARDIO MACHINES – Sized for a standard treadmill footprint with room to spare. Rolls out flat with no curling at the edges, rolls back up when you need the space.
- LIGHTWEIGHT, WIPES CLEAN, GOES ANYWHERE – Light enough to move with one hand, dense enough to stay put. Wipe it down with a damp cloth. Works just as well under a weight bench, in a playroom, or on a workshop floor.
A careful account of a family tragedy
The confirmed facts are stark but limited: Exodus Tyson was four years old; a treadmill cord or cable became caught around her neck at a Phoenix-area home; her brother found her; her mother called 911 and attempted CPR; emergency responders treated her; and she died in hospital the next day. Police described the event as accidental and initially found nothing suspicious.
The available reporting does not justify more graphic language, a claim of intentional self-harm, an electrocution claim, or an accusation against the family. Respecting those limits keeps the account focused on what is documented—and on the practical responsibility of keeping children separated from exercise equipment and its cords.
Frequently Asked Questions
How did Mike Tyson’s daughter Exodus die?

Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsDo we know exactly how the treadmill accident happened?
The reports reviewed do not establish the exact movement that caused the entanglement. Police offered possibilities including slipping or placing her head into a loop beneath the treadmill console.
Would a treadmill safety key have prevented Exodus Tyson’s death?
No. A safety key can stop a treadmill in an emergency, but it is not a substitute for supervision, physical separation, secure cord management, or following the model-specific manual. There is no evidence that the treadmill involved in Exodus’s death used a particular safety-key design or that a replacement key would have prevented the accident.
The Bottom Line
Exodus Tyson died on May 26, 2009, after a cord attached to a treadmill became caught around her neck at a Phoenix-area home the day before. Phoenix police described the event as a tragic accident and did not report suspicious circumstances. The exact mechanics remain uncertain; the responsible safety lesson is supervision, physical separation, secure cord management, and proper use of the treadmill’s emergency-stop system—not speculation or blame.
