Danny Simms and Xander Phillips do not officially get back together at the end of Pulse. They do, however, reach the most honest and emotionally settled point of their relationship: they admit that they love each other, confront the secrets and power imbalance that damaged them, and apologize. The finale stops short of calling that a romantic reconciliation because Xander is becoming an attending while Danny remains a resident under the same hospital system.
At the same time, Xander finally reports the truth about Julio’s death at Kennedy to the hospital review board, risking his medical license. Danny loses the chief-resident position to Sam, but remains at Maguire. Her final swim in the ocean is not a promise to Xander—or a rejection of him forever. It is Danny choosing freedom and self-possession before making another decision about her career or relationship.

What happened between Danny and Xander?
Danny and Xander’s relationship was genuine. The flashbacks show that it was not merely a casual affair or a plot device created to explain the tension between them in the present-day story.
Xander arrived at Maguire Medical Center about a year before the hurricane. He and Danny gradually became close, began a serious relationship after a donor gala, exchanged keys to their homes, and established routines such as Sunday dinners. They were emotionally invested in each other well before their professional relationship became impossible to ignore.

The problem was that Xander was Danny’s chief resident and mentor while she was competing for advancement within the emergency-medicine program. His professional authority could affect her reputation, opportunities, and future at the hospital. Even though the relationship included real affection and apparent consent, the hierarchy made it difficult for Danny to know whether her career would be judged independently of Xander.
That conflict became central when Xander was offered an attending position at Maguire. Xander wanted them to be more open about their relationship. Danny believed disclosure could cost her the chief-resident position and lead colleagues to assume that her achievements were the result of favoritism. Xander saw her reluctance as evidence that she was pulling away. Danny saw his pressure as a willingness to put his own emotional needs—and perhaps his professional transition—ahead of her career.
They said “I love you” at the retreat shortly before Danny filed her HR complaint. That detail matters: the show does not present the relationship as fake simply because Danny later reported it. Danny could love Xander and still decide that a relationship with her superior had created an unsafe or coercive professional situation.
Why did Danny file the HR complaint?
Danny’s complaint was an attempt to regain control over a relationship in which private feelings and institutional power had become inseparable. She wanted an impartial authority to acknowledge that Xander’s position over her created a professional problem, even though she had participated in the romance and continued to have feelings for him.
The complaint is therefore not best understood as either a fabricated accusation or a straightforward villain verdict against Xander. Pulse deliberately allows two facts to coexist:
- Danny and Xander’s relationship contained real love and mutual intimacy.
- Xander’s authority over Danny made the relationship professionally compromising and difficult for her to navigate freely.
Danny also has a broader tendency to compartmentalize her emotions. She tried to keep her ambitions separate from her love for Xander, but the hospital structure made that separation impossible. Her complaint was partly self-protection and partly an attempt to force the institution to recognize a conflict she could no longer manage privately.
By the finale, Danny drops the complaint with Harper’s support. That decision clears the way for a more direct conversation with Xander, but it does not prove that the complaint was baseless or erase the underlying power imbalance. Dropping it gives Danny a different kind of agency; it does not rewrite what happened.
What did Xander do at Kennedy?
The finale reveals that Xander did not leave Kennedy because of an inappropriate workplace relationship, as the show initially encourages viewers to suspect. The secret concerns a patient death and the way Xander’s powerful family helped conceal it.

Before joining Maguire, Xander treated Julio, a young patient who arrived at Kennedy with severe abdominal fluid buildup. Under pressure to move patients through the emergency department, Xander performed a paracentesis without ultrasound guidance. The procedure initially seemed successful. Julio later returned unconscious and began bleeding.
Xander realized that he had probably nicked a blood vessel during the procedure. Julio died in front of him.
The distinction between accident and intention is important. The series does not present Xander as someone who intentionally killed Julio. It presents a rushed, unsafe procedure that led to a fatal complication, followed by a cover-up that prevented Xander from facing the professional consequences openly.
Xander’s parents used their influence and family name to suppress the incident. The people involved signed nondisclosure agreements, allowing Xander to continue his medical career while leaving him with guilt, fear, and unresolved trauma.
That trauma explains his repeated panic when later procedures resemble the Kennedy case. It also helps explain why he freezes during Catherine’s surgery. Danny’s intervention allows him to resume treatment, but the moment makes clear that the original incident has never actually been dealt with.
Why does Xander confess to Danny?
Xander first tells Danny the full story because the NDA had prevented him from explaining why he left Kennedy. His confession changes what Danny understands about his secrecy. She had been left to assume that his departure was connected to the workplace relationship or another undisclosed professional issue. Instead, she learns that he has been carrying the memory of a patient’s death and the consequences of his family’s intervention.
But the more consequential confession comes afterward. Xander tells Danny that he has reported himself to the hospital review board. He may lose his medical license.
This is not presented as a grand gesture intended to win Danny back. Xander is not confessing because he expects Danny to reward him with a relationship. He is finally choosing professional accountability over his family’s protection and his own career.

That decision marks his clearest change in the finale. Earlier, Xander repeatedly protected himself through secrecy, pressure, and institutional privilege. Now he accepts that transparency may destroy the career his family helped preserve. He tells Danny that he wants to become more like her, making her ethical influence part of his turning point—but the decision still has to be his own.
Who becomes chief resident?
Danny appears to be the leading candidate for chief resident after Natalie Cruz supports her. Cruz also refuses pressure from Xander’s parents to remove Danny from the residency program.
Nevertheless, Danny does not get the job. After Patrick becomes chair of the emergency department, he names Sam Elijah as chief resident instead. Danny, Harper, and Natalie are all shocked by the decision. Sam returns to the hospital and begins settling into the leadership role.
The outcome is a professional loss, but Danny is not expelled from Maguire. She remains a resident and continues her medical training. The finale uses that distinction to separate Danny’s worth from the institution’s approval. She does not receive the title she fought for, yet she is not reduced to a failed candidate or defined by Xander’s influence over her career.
Danny’s response is also significant. Rather than treating Sam as a personal enemy, she ultimately embraces him. Her identity begins to move beyond the promotion, the complaint, and the question of whether Xander’s presence has shaped every judgment made about her.
Are Danny and Xander together at the end?
Not officially. They are emotionally reconciled, but the finale does not confirm that they resume dating.
In their first major conversation in the finale, Xander tells Danny what happened at Kennedy, acknowledges that their relationship put her in an uncomfortable position, and apologizes. They both say that they love each other. The scene resolves some of the misunderstandings that have kept them apart, but it does not magically make their relationship safe or uncomplicated.
The central obstacle remains visible: Xander is moving into an attending role while Danny is still a resident. Even if their personal feelings are sincere, he continues to occupy a position of professional authority within the same institutional world. The finale leaves the audience with a question rather than an answer: can they ever have a healthy relationship without that hierarchy distorting their choices?

The most accurate description is that Danny and Xander clear the emotional slate without restarting the romance. They still love one another. They understand each other better. They are not shown making a commitment to be a couple.
What does Danny’s final swim mean?
Danny’s final swim is the visual expression of the ending’s central idea: she needs to recover her freedom before choosing what comes next.
For most of the season, Danny is surrounded by emergencies, professional scrutiny, family trauma, hurricane damage, and pressure from both the hospital and Xander. In the ocean, she is temporarily separated from all of those demands. She has no title to win, complaint to defend, romance to define, or institution watching her make the correct choice.
The scene is best read as release and catharsis. Danny enters the water alone, but that does not mean she has rejected Xander forever. Xander has just taken responsibility for his past, and their relationship is more honest than it was earlier in the season. Her solitude means that her next step does not have to depend on whether Xander stays in her life.

It also does not represent suicide imagery or a literal escape from Miami. The scene is framed as Danny surrendering, briefly and deliberately, to something she cannot control. She can love Xander without immediately returning to him. She can lose the chief-resident job without losing herself. She can continue her residency without allowing the hospital to determine her entire identity.

Pulse was written with possible future storylines in mind. The finale leaves open several major questions:
- Will Xander keep his medical license after reporting Julio’s death?
- What will his new attending role mean for his professional relationship with Danny?
- Will Danny and Xander eventually try to build a relationship outside the hospital hierarchy?
- How will Sam handle being chief resident?
- What will Patrick’s leadership mean for Maguire Medical Center?
Those questions are no longer waiting for a produced second season. Netflix canceled Pulse after its first season in July 2025, so there is no Season 2 that resolves Xander’s medical-board consequences, Danny and Xander’s romantic future, or the hospital’s changing leadership.
Viewers who want to revisit the clues can watch Pulse on Netflix, where the ten-episode first season establishes the relationship, the Kennedy mystery, and the institutional pressures that make the ending intentionally cautious. There is no second season to add a conventional romantic or professional resolution.

The ending’s real meaning
The finale contrasts emotional truth with institutional reality. Danny and Xander’s love is real, but love does not erase a reporting relationship, career pressure, secrecy, or family power. Their honesty is an important improvement, not a complete solution.
Xander’s decision to report himself is the season’s clearest act of accountability. Danny’s decision to swim alone is her clearest act of independence. Together, those choices suggest that both characters must become more honest and autonomous before a healthy future—romantic or otherwise—could be possible.
So the ending is neither a clean breakup nor a happy-ever-after reunion. Danny and Xander still love each other, but they do not officially get back together. Xander accepts responsibility for Julio’s death, Danny loses the chief-resident role to Sam, and Danny’s final swim confirms that her immediate future belongs to her.
