The short answer: Skye never escapes
The apparent final victory is another trap. Skye believes she has escaped the wellness center, killed her abusive mother Elizabeth, reunited with Gemma, found Morris, and discovered a way to stop the entity by temporarily stopping her heart. None of that gives her a genuine way out.
The film eventually returns to the real-world concert at Madison Square Garden. Elizabeth is still alive in the audience, proving that Skye’s hospital confrontation and escape did not happen as she experienced them. Skye is still onstage, surrounded by fans and cameras, and the Smile entity has already gained control of her perception and body.

A monstrous smiling version of Skye appears. The entity enters Skye through her mouth, and Skye impales herself in the face with her microphone while the audience watches. The final image is therefore not Skye choosing a successful escape. It is the entity using Skye’s body and public performance to stage a spectacular death.
What was real and what was the entity’s illusion?
Smile 2 deliberately refuses to identify one clean moment when reality ends and hallucination begins. The safest interpretation is that the entire late escape sequence is unreliable, rather than that the movie gives us a precise, easily marked transition.
Elizabeth’s apparent death
Skye believes she kills her mother, Elizabeth, during the hospital rampage. But Elizabeth’s presence in the concert audience makes clear that this death did not occur in the film’s final reality. Morris and the heart-stopping plan
Morris proposes a theory based on briefly stopping the infected person’s heart and then reviving them in isolation. The idea is that the victim could technically die without anyone witnessing the traumatic event, preventing the curse from passing to another person.
The film presents this as a possible strategy, not a confirmed cure. The later freezer sequence is exposed as another part of the entity’s deception, so Skye never proves that the procedure works. Morris’s ultimate status is also left deliberately uncertain. He may have been a real person who researched the entity and briefly contacted Skye, or the entity may have fabricated more of his role than the film confirms.

Why does Skye die onstage?

Does everyone at the concert catch the Smile curse?

This also explains why the concert is such an effective final setting. Skye’s entire public identity is built around performance. Onstage, she is expected to smile, remain composed, and give the audience a version of herself they can consume. The entity turns those expectations against her. Her most public moment becomes the place where her private terror is exposed and her body is used for the final spectacle.

Does Skye choose to kill herself?
Skye’s final action is not best understood as a free, informed decision. The entity has already taken control of her perception and body. The monstrous smiling version of Skye emerges before she attacks herself, showing that the supernatural force is actively directing what happens.
In plot terms, the entity causes Skye’s death. In thematic terms, it weaponizes her own body, career, and performance against her. How the ending connects to the first Smile
