Who played Frank Costanza first?
John Randolph played Frank in the original broadcast of “The Handicap Spot.” Jerry Stiller later recalled, “John Randolph dropped out of the role, and I was needy enough to do an interview to replace him.” An episode reference identifies Randolph’s appearance as Frank’s first and says Stiller formally began playing him the following season.

A contemporaneous 1994 Los Angeles Times profile adds context: Stiller initially passed on the part because he had a play to do, then was asked again after the production ended.
Why did Jerry Stiller’s version of Frank change?
Stiller said the producers initially envisioned Frank as quiet and meek, in contrast to Estelle Costanza, played by Estelle Harris. During rehearsals, Stiller began shouting back at Estelle and made Frank more forceful. He recalled that the crew laughed when he slapped George on the head.

In a 2005 interview excerpt republished by Esquire, Stiller described asking Larry David to change his approach: “On the fourth day, I said to Larry David, ‘This ain’t workin’. Can I do it my way?’”
Those are Stiller’s recollections of how he developed the role. They explain the creative shift viewers see in his Frank, but they do not establish that Randolph was dismissed for poor performance or that a conflict prompted the recasting.

Why are there two versions of “The Handicap Spot”?
The original season-four broadcast featured Randolph. According to the episode reference, the Randolph scenes were reshot with Stiller in spring 1995 for U.S. syndication. Stiller’s regular run as Frank began the next season.
Consequently, the actor playing Frank depends on which version of the episode a viewer sees: Randolph in the original broadcast, or Stiller in the syndication reshoot. The reference reports that both versions appear on the Season 4 DVD set, but that detail may depend on the specific edition; check the contents of the exact release before buying it to compare them.
