A frightening villain can shock the audience; an effective antagonist changes what the story means. Johan Liebert does both. In Naoki Urasawa’s Monster, Johan’s crimes drive a mystery, but his deeper force comes from the moral pressure he places on Dr. Kenzo Tenma and from the unanswered question of what makes someone a monster. This is a critical judgment, not a claim that any definitive ranking exists. Spoilers follow for the series.
Why Johan’s conflict with Tenma matters
VIZ describes Monster as a thriller about a brilliant doctor’s journey into the human soul, involving conspiracy, serial murder, and questions of good and evil. Its first-volume synopsis sets the central dilemma in motion: Dr. Kenzo Tenma chooses to save a patient rather than protect his professional standing, then faces the consequences of that decision. The publisher presents Johan as a calculating killer with a mysterious past, and says that while his identity is known, the reasons for his killings remain unknown. That premise gives Johan a relationship to Tenma beyond pursuit and escape. Tenma’s choice begins as an expression of professional duty and respect for human life; the consequences force him to confront what that principle demands when the life he saved appears to have become a source of terrible harm. Johan therefore challenges Tenma’s values without making the choice to save a patient automatically wrong. The tension is not simply whether Tenma can stop a killer, but whether he can preserve his conviction that every life matters while confronting what followed from his decision.
How Johan shapes the story even when absent
An antagonist earns narrative weight not only through scenes in which they appear, but through the choices and consequences they set in motion. VIZ’s synopsis makes Johan the center of a mystery about Tenma’s decision and the killer’s unknown motives. That structure lets Johan influence the story across gaps in his presence: other people’s experiences, Tenma’s investigation, and the search for an explanation all circle back to him.

The result is a particular kind of suspense. The reader may know who Johan is while still lacking a satisfying answer to why he acts as he does. Mystery does not merely conceal a fact until a reveal; it keeps the character’s meaning unsettled. Each new piece of information can alter how the audience understands the violence, the people around him, and Tenma’s pursuit.
Why the mystery of motive is more unsettling than a crime tally
Johan’s crimes establish danger, but a list of terrible acts would not by itself make him a compelling antagonist. The uncertainty around his reasons gives those acts narrative and moral force. If the story offered a simple explanation that reduced him to a familiar motive, the central question would narrow. Instead, the gap between what Johan does and what can be confidently said about why he does it keeps the audience interpreting rather than merely counting harms.

This uncertainty should not be mistaken for an excuse. Asking how Johan understands himself is not the same as denying the damage his actions cause. It makes the antagonist more difficult to dismiss as an inhuman exception, while leaving accountability intact. The distinction between explanation and justification is essential to the story’s moral tension.
What “monster” asks us to consider
Urasawa has discussed the title in terms of Frankenstein’s creature: a being created by humans and rejected despite not choosing the circumstances of its existence. In a French-language interview, he also reflects on how people regarded as evil may understand their own actions as just. That context complicates the word “monster.” It can name someone who commits horrific acts, but it can also point toward the conditions and judgments that shape how people see themselves and one another. Urasawa’s comments do not resolve the question by declaring Johan a victim or by absolving him. Rather, they help explain why the series keeps the boundary between human and monstrous under examination.
Why Johan stands out as an antagonist
On four useful qualitative measures, Johan makes a strong case for being one of fiction’s best antagonists:

- Influence on the plot: The publisher’s premise ties the mystery of Johan and the consequences of Tenma’s choice together, so the antagonist is central to the story’s direction.
- Pressure on the protagonist’s values: Johan’s existence makes Tenma’s commitment to saving lives painfully consequential without reducing that commitment to a mistake.
- Uncertainty and revelation: The known identity and unresolved motives create sustained suspense around what the audience can truly understand.
- Thematic fit: Johan’s role connects the thriller’s pursuit of a killer to questions about evil, rejection, responsibility, and the meaning of “monster.”
These are interpretive criteria, not a scoring system or proof of a universal consensus. They explain why Johan’s impact extends beyond menace: his character links plot mechanics to the ethical and thematic concerns of Monster.

The series supports the question without requiring a single answer. Johan can be judged by the harm he causes, while the story still asks how a person comes to understand their actions and how rejection or circumstance relates to identity. Urasawa’s remarks make that inquiry more pointed, but they do not establish one final account of Johan’s psychology.

Holding both ideas at once is part of the character’s power. If Johan were only an embodiment of evil, the thematic question would close too quickly. If the story treated his circumstances as an excuse, the moral stakes would weaken. Instead, the uncertainty asks the audience to consider causes and self-understanding without confusing them with innocence.
Does Johan deserve to be called one of the best antagonists ever?
“Best” depends on the criteria and the comparison set; no objective ranking can settle the question. By the standards used here—shaping the plot, testing the protagonist’s moral commitments, sustaining uncertainty, and embodying the work’s themes—Johan earns the claim. He is frightening not only because of what he does, but because the story makes his presence a continuing question about Tenma’s choice and the possibility of understanding evil without excusing it.
