Why is she called “Crazy Eyes”?

In a 2016 ABC News interview, Aduba cautioned against reducing her to the nickname: “She’s ‘Crazy Eyes’ to the world. But she’s Suzanne when you get to know her.” Aduba also said, “She’s unique.”

Netflix says the series is based on Piper Kerman’s 2010 memoir, Orange Is the New Black: My Year in a Women’s Prison. That connection is the series’ inspiration, not evidence that Suzanne’s fictional crime appears in Kerman’s account. Netflix’s retrospective identifies the memoir as the series’ basis.
