Butcher sees Becca as a hallucination, not because she has physically returned. In his mind, she urges him to protect Ryan and represents the conscience and humanity he is struggling to hold on to while gravely ill and under pressure.
Is Becca really back?
No. Becca appears in Butcher’s mind; the story does not establish that she has returned as a physical presence. Her appearances are emotionally real to him, but they reflect his inner conflict.
The context is a crisis: Amazon’s Season 4 overview says Butcher has only months to live, has lost Ryan, and has lost his position as leader of the Boys.
What does Becca want Butcher to do?
In the Season 4 premiere, Becca urges Butcher to save Ryan from Homelander’s influence and to be the man she married. Her presence connects Butcher’s choices to the person he wants to be and the family he has lost. TVLine’s premiere interview describes her appeal to him about Ryan.

Why does Butcher see both Becca and Kessler?
Showrunner Eric Kripke explained that the writers used the two figures to dramatize the opposing sides of Butcher’s struggle: his human impulses and his darker, more destructive desires. Kripke described the idea as “an angel and a devil on Butcher’s shoulder,” not as literal supernatural beings. Becca embodies his conscience; Kessler represents his darkest impulses. Kripke’s interview with TheWrap discusses the device.
Episode 6 makes the parallel explicit by revealing that Kessler is also a hallucination. When Kessler acknowledges Becca, Butcher realizes that neither figure is an independent person in the scene. Both are part of his internal experience, pulling him in different directions.

What the visions mean for Butcher
Becca’s appearances give the show a way to stage Butcher’s conflict rather than simply explain it: she presses him toward protecting Ryan, while Kessler gives voice to his darker side. His illness and precarious circumstances form the backdrop, but the point of the visions is also emotional and moral—Butcher is fighting over what kind of person he will be.
