Monica Beltran dies because of injuries from the hospital explosion, but the creative reason for her death was what it would mean for the people she saved. Showrunner Meg Marinis said Monica’s final act—guiding Jules Millin through an operation that saves a child—would shape Jules’s and Amelia Shepherd’s stories going forward.
What happens to Monica
The season 21 finale leaves the aftermath of a hospital explosion unresolved. In the season 22 premiere, Monica is trapped with Jules and a child who needs surgery. Though badly injured, Monica helps Jules finish the operation. Monica dies before rescuers can reach them; Forbes reports that she suffers internal bleeding from her injuries (Forbes, October 10, 2025).
Why the writers chose Monica
Marinis explained that the writers considered how each possible death would affect the surviving characters and carry into the next season. Monica’s death gives Jules a defining moment: she completes a major operation with Monica’s guidance, saving the child. It also leaves Jules and Amelia with grief that can continue to shape their stories.
Marinis said the choice emerged as the operating-room scenes took shape. She described Monica’s final moments as selfless: Monica keeps teaching and guiding Jules while helping save both Jules and the child. “I wanted to see someone die selflessly saving someone’s else’s life, and she was not only saving that little kid on the table, she was saving Jules,” Marinis told TVLine on October 10, 2025.

Marinis also told TV Insider on October 9, 2025, “We don’t just kill characters off to kill characters off. We obviously want it to lead to interesting storytelling for other characters.” She said Monica’s death “just is where the story organically led.” Marinis has also described the decision as painful and said she did not want to lose Monica.
Was Monica’s death an actor or production decision?
The explanation Marinis gave is a story decision: Monica’s death enables her final act of mentorship and gives Jules and Amelia consequential stories in the aftermath. TVLine reported that Natalie Morales understood the storyline and was not too surprised, but that reported reaction does not establish a separate reason for the character’s death. The available reporting does not support claims that Morales requested to leave, was fired, or was targeted for an off-screen reason.

Monica and Amelia’s romantic tension is part of the emotional context, but Marinis said the decision was not intended to invoke the “Bury Your Gays” trope, as reported by TVLine. Reducing the choice to their relationship would miss the showrunner’s stated emphasis on Monica’s sacrifice and the effect on both Jules and Amelia.
