“Cause” and “manner” are distinct parts of the official finding: mixed drug toxicity describes the medical cause, while “accident” is the certified manner. The county’s wording does not apportion the combined toxicity among the substances or say that fentanyl, cocaine or alcohol alone caused his death.
When and where did he die?
McCormick was found unresponsive at his home in Studio City on September 7, 2018, and was pronounced dead at the scene at 11:51 a.m., according to the county’s November 5, 2018 release. An autopsy was conducted the following day. The cause was initially deferred while the investigation continued, before the county issued its final determination.

What did prosecutors say about counterfeit pills?
In October 2019, federal prosecutors announced charges against three men in a scheme involving narcotics and counterfeit pharmaceutical pills containing fentanyl. The Department of Justice said the indictment alleged that pills distributed in connection with McCormick’s death caused an overdose. That account is a prosecution allegation in a charging document, not the Medical Examiner’s cause-and-manner determination. The county’s formal finding remains mixed drug toxicity involving fentanyl, cocaine and ethanol, with the manner certified as an accident.
