What to do first
- Set the medicine aside. Do not take another dose, taste it, or try to identify an unfamiliar substance. Keep it separate from medicines you believe are safe. Avoid opening or altering a suspicious package further.
- Check whether anyone was exposed. If someone may have taken the medicine or otherwise been exposed, contact Poison Control or a hospital for case-specific guidance. If the person is having a medical emergency, call 911.
- Arrange a safe prescription plan. If the medicine is prescribed, promptly call the dispensing pharmacist and prescriber. Ask how to obtain a safe replacement and how to avoid an unsafe interruption. There is no single substitute-dose plan that applies to every medicine, so do not guess or double a later dose without professional advice.
- Keep the evidence. Retain the medicine and original packaging, including labels, seals, lot numbers, and other codes. Do not discard them before speaking with the pharmacist, retailer, manufacturer, or regulator.

If no one has been exposed and there is no emergency, leave the product unused while you seek advice. The right next step depends on the situation: damaged tamper-evident packaging on an OTC medicine calls for contacting the store, while a concern about a prescription calls for prompt advice from the dispensing pharmacist and prescriber. A suspicion of criminal counterfeit activity has a separate FDA reporting route, but it should not delay medical help or urgent exposure advice.
How to recognize warning signs—and what they cannot prove
Inspect the medicine and package
For over-the-counter (OTC) medicines, check the tamper-evident features described on the label. Inspect the outer package before buying and the medicine again after bringing it home. Do not use a product with damaged packaging, or medicine that is discolored or otherwise looks different than expected. These checks are useful precautions, not a guarantee. Tamper-evident packaging is not completely tamper-proof. If you find a broken seal, damaged container, or other change that makes you unsure whether the product is safe, do not use it simply because the medicine itself looks normal. Preserve the package and ask the retailer or a health professional what to do.
Notice changes in a prescription product
FDA lists packaging or medicine that looks different from what a person normally receives, a new or unusual side effect, and purchase from an online pharmacy among possible signs of counterfeit medicine. Treat these as reasons to stop and seek professional advice—not as proof that the product was tampered with. FDA guidance on counterfeit medicine
If a prescription looks different, note exactly what changed and when you noticed it. Tell the pharmacist where it was dispensed and provide the package and product details. Appearance is only one clue: a new or unusual effect also deserves professional attention, but it does not by itself establish the cause or show that a product was altered.

Do not rely on a home test
A visual concern cannot confirm that tampering or contamination occurred, and a normal appearance cannot establish that a medicine is safe. Legitimate pills may look different between manufacturers or refills; appearance alone is not a reliable test. FDA also cautions that tamper-evident packaging is not completely tamper-proof. The reviewed guidance does not identify a reliable home test for tampering or contamination. When in doubt, leave the medicine unused and ask a pharmacist or prescriber what to do rather than trying to inspect, taste, or identify the contents yourself.

This is an action guide, not a way to determine from a photograph or appearance whether a medicine is safe. If you cannot tell which situation applies, the immediate rule is the same: do not use the suspect product, preserve it, and ask a pharmacist or prescriber for guidance. If exposure may have occurred, make that the priority and contact Poison Control or a hospital; use 911 for a medical emergency.
How to report a medication concern in the United States
1. Notify the seller and the company on the label
Tell the retailer where the product was purchased and the manufacturer or distributor identified on its package. For an OTC product that looks suspicious, FDA specifically advises contacting the store. FDA’s consumer guidance puts it plainly: “If anything looks suspicious, be suspicious. Contact the store where you bought the product. Take it back!” Do not return or handle the product in a way that destroys useful packaging or identifying information before getting advice.

2. Submit a non-emergency report to FDA
FDA directs consumers to MedWatch for non-emergency medication quality and safety concerns, including problems involving authenticity, contamination, packaging, or labeling. The FDA consumer complaint system also includes suspected product tampering as a complaint type. These reporting routes are for product concerns; they do not replace urgent medical care or immediate advice after an exposure. FDA lists a separate Office of Criminal Investigations path for suspected criminal counterfeit activity. If that is your concern, preserve the product and identifying information, but put treatment and exposure advice first. A non-emergency report should not hold up a call to a pharmacist, prescriber, Poison Control, hospital, or 911 when those are needed.

- Product name and type, plus the lot number or other package codes and identifying marks.
- Where and when it was purchased, and the seller’s details if available.
- What looked or seemed wrong and when you noticed it.
- Whether anyone was exposed, what happened afterward, and whether a clinician treated the person.
- Relevant contact information for the affected person or clinician, if applicable.
FDA asks for a clear account, product identifiers, affected-person and clinician details where relevant, and seller and purchase information. Be as specific as you can about the concern—such as a damaged seal, an unusual appearance, or a change in the product—without presenting a suspicion as a confirmed finding. Keep the product and original package available in case the pharmacist, retailer, manufacturer, or regulator needs to examine the details. FDA: how consumers can report a problem
What not to do
- Do not take another dose as an experiment. It may expose you to more risk and will not reliably identify what is wrong.
- Do not taste or handle an unknown substance to identify it. Leave the product unused and seek professional guidance.
- Do not make a prescription change on your own. Ask the pharmacist or prescriber how to bridge treatment safely rather than guessing at a replacement, skipped dose, or altered schedule.
- Do not assume suspicious means confirmed tampering. A difference in appearance, unusual effect, or damaged package is a warning sign that merits action, not proof of a crime or contamination.
- Do not throw away the package or product immediately. Its codes and condition can help the people assessing or investigating the concern.
There is no product or household check that can establish that a suspicious medicine is safe. A pill organizer, storage container, or identifier cannot resolve a concern about tampering. Keep the medicine apart from products you consider safe, avoid further handling, and use the relevant professional and reporting channels instead.
