Val Kilmer’s appearance changed in the years around his throat cancer and treatment, but the available reporting does not identify a specific facial operation or explain every visible difference. It documents chemotherapy, radiation, two tracheotomies, a period when he was described as thin, and lasting effects on his voice. The tracheotomy and scars were around his neck; they should not be mistaken for evidence of facial surgery.

What happened to Val Kilmer’s face?

Kilmer’s changed appearance was discussed in the context of his illness and cancer treatment. A 2017 ABC News report described him as thin during that period and noted that his voice was quiet and his speech difficult to understand. That dated observation does not establish one cause for every later change in his appearance. ABC News reported on his appearance and treatment in December 2017.

Later accounts describe a tracheotomy and surgical scars, particularly around his neck. The sources do not establish that he had a facial procedure, nor do they support diagnosing specific facial changes from photographs. It is more accurate to distinguish documented treatment effects from conclusions that reporting does not substantiate.

What treatment did he undergo?

The Associated Press reported that Kilmer was diagnosed with throat cancer in 2014 and that his treatment included two tracheotomies. Accounts of the documentary Val also describe extensive radiation and chemotherapy. Kilmer’s own words, narrated by his son Jack in the documentary, were: “I was recently diagnosed with throat cancer, though I healed quickly from the cancer. The extensive radiation and chemotherapy that followed treatment has left my voice impaired.” This is Kilmer’s personal account, not an independent medical assessment. ABC News Australia covered the documentary and his account; the Associated Press obituary reports his diagnosis and tracheotomies.

Why did he look different in later years?

The reporting supports several pieces of context, not a single explanation for every change:

  • Thinness during treatment: ABC News described Kilmer as thin in its 2017 report. That describes his appearance at that time; it is not a diagnosis or a complete account of his later appearance.
  • Tracheotomy and neck scars: The tracheotomy changed how he spoke and helped him breathe, according to a 2020 interview report. Later coverage describes scarves covering his tracheotomy or surgical scars. ABC News reported on the tracheotomy and his memoir. TIME also noted a scarf over scars in Top Gun: Maverick in its 2025 remembrance: TIME’s remembrance of Kilmer.
  • Voice and speech effects: Coverage of Val and his 2020 interviews describes an impaired voice and changed speech. This is the most consistently documented lasting physical effect in the accounts cited here.

In a 2020 Good Morning America interview, Kilmer said, “I’m doing great, I feel a lot better than I sound.” That personal remark conveys how he described himself at the time; it is not a medical evaluation. ABC News reported the interview.

What is known about his death?

Kilmer died in 2025 from pneumonia, according to the Associated Press report citing his daughter. That reported cause of death should be kept distinct from his earlier throat cancer diagnosis; the AP account does not identify throat cancer as the cause of death. The Associated Press obituary provides that account.
