Yes. Sweeny explicitly confirmed Moriarty’s death after the finale, calling it “the conclusion of his time as a human on this planet.” He also said that Moriarty’s actions and the things he caused Watson to do would continue to hover over the series. Review: a doctor treating the person he cannot safely save
The moral conflict is the finale’s strongest material
The episode’s most consequential choice is not simply whether Watson can solve the medical crisis. It is what he owes a patient who remains a threat. Watson acts as Moriarty’s doctor, but he is also the person who decides not to take further steps to prevent his enemy’s death. That makes the bedside scene an uncomfortable collision between the duty to treat and the obligation to protect others.
Sweeny’s explanation is that Watson was trying to minimize harm: Moriarty still had the means to endanger people, and Watson believed his decision would limit the damage. Sweeny said the choice would have ramifications for Watson, but that he did not think Watson would change what he did. That rationale gives the decision a clear ethical frame, without making it uncomplicated. (TV Insider)

Does the two-part plot pay off?
The Croft twins’ illness and the one-vial dilemma give the finale an urgent procedural problem, while Moriarty’s infiltration makes the danger personal for Watson’s team. The plot ultimately ties its medical solution to Moriarty himself, so the crisis and the final confrontation are not separate threads. The recap lays out how the episode develops a response to the twins’ danger and carries the consequences of Moriarty’s scheme into Watson’s choice. (TVLine recap)
Does the ending give the scene enough weight?
Moriarty’s death is staged as a personal reckoning rather than a triumphant takedown. Watson stays with him, and the two-week-later image of the shirt offers a subdued sign of closure. It also leaves a useful question for the series: if Moriarty is gone, what does Watson do with the compromises and consequences left behind? The finale points toward that aftermath instead of suggesting the conflict disappears with its antagonist.
