Tom Petty died on October 2, 2017, at age 66. The Los Angeles County medical examiner ruled his death an accident caused by mixed drug toxicity, with the official cause recorded as multisystem organ failure following a resuscitated cardiopulmonary arrest. His family separately said severe pain from a worsening hip fracture contributed to medication overuse.
What was Tom Petty’s official cause of death?
The Los Angeles County Department of Medical Examiner-Coroner recorded the cause as “multisystem organ failure due to resuscitated cardiopulmonary arrest due to mixed drug toxicity” and classified the manner of death as accident. The wording describes a chain of events: mixed drug toxicity led to cardiac arrest that was resuscitated, followed by multisystem organ failure. The report is available in the medical examiner’s autopsy report.
“Accidental overdose” is a common shorthand for the finding, but the report’s more specific language is mixed drug toxicity and its manner-of-death classification is accident.
Which substances and other conditions did the report list?
Substances
The report listed seven substances: fentanyl, oxycodone, temazepam, alprazolam, citalopram, acetylfentanyl, and despropionyl fentanyl. That toxicology list does not establish that every substance was prescribed to Petty. In contemporaneous coverage, Los Angeles County coroner spokesman Brian Elias said, “A lot of these are found in prescription drugs”—a general comment, not a case-specific account of where each substance came from.

Other recorded conditions
The report also noted coronary artery atherosclerosis and emphysema as other conditions. These were recorded alongside the cause of death; the report’s stated cause remained mixed drug toxicity.
What did Petty’s family say about his hip and medication?
In a statement reported by the Los Angeles Times, Dana Petty and Adria Petty said Tom had emphysema, knee problems, and a fractured hip. They said he toured 53 dates with the injury, which worsened, and learned on the day he died that it had become a full break. The family said he had been prescribed pain medication for multiple issues, including fentanyl patches.

The family believed the pain contributed to medication overuse, calling his death an unfortunate accident. They wrote that, in their view, “the pain was simply unbearable and was the cause for his over use of medication.” That is the family’s explanation of the circumstances; it is distinct from the medical examiner’s official determination of mixed drug toxicity.
