There is no reliable basis in the available official records for ranking ten theme park accidents as the “worst” in history. The documented cases below are not a top-three list: they illustrate different kinds of harm and different limits in what an incident record can establish. One involved a ride car falling; two involved employees struck by operating roller coasters. The lessons are about what the records support—not claims that these cases represent the typical visit or that a particular safety change followed.
Why this is not a ranked list of ten
“Worst” can refer to deaths, injuries, the seriousness of a failure, or the lasting consequences. Those are distinct measures, and the official records available here do not provide comparable findings for ten incidents. The three examples below therefore have no numerical ranking.
The U.S. Consumer Product Safety Commission’s 2000 report covers U.S. amusement-ride fatality data for 1987–1999 and injury data for 1993–1999; it is not a global ranking of individual accidents. Read the CPSC report.
A separate CPSC review examined records for 90 investigated incidents from January 1, 1990, through June 21, 2001. It identified hazard patterns involving mechanical failure, operator behavior, consumer behavior, other factors, and combinations of factors. The cases were neither a probability sample nor a complete account, so those categories should not be read as estimates of how often each cause occurs. Read the CPSC incident review.

Three documented accidents and the lessons their records support
Dallas Enterprise ride, October 17, 1983
A CPSC alert dated November 1, 1983, reported that a gondola car on an Enterprise ride became dislodged and fell to the ground. One person was killed and a number of others were injured. At the time of the alert, the Commission said it was conducting a disassembly and engineering analysis and did not know the specific cause. The alert documents the event and its toll, but it does not establish why the car became dislodged. Read the CPSC alert.
What the record teaches: distinguish a documented failure from an established cause. Without the completed engineering findings, it would be misleading to attribute this accident to a particular design defect, maintenance error, or operator action.

Six Flags Magic Mountain, April 9, 2004
OSHA’s incident summary says an employee performing safety checks for the Scream roller coaster entered a restricted infield area while the ride was operating and was struck by a train. The employee suffered blunt-force head trauma and died. OSHA’s summary attributes the accident to the employee’s presence in the restricted area while the ride was operating; it is not a full independent engineering investigation. What the record teaches: safety checks around an operating ride require clear separation between people and moving equipment. The summary establishes the employee’s presence in a restricted area and the collision, but it does not provide enough detail to infer what barriers, procedures, or controls were in place or to prescribe a specific corrective change.

Six Flags Great America, May 29, 2004
OSHA’s record says a maintenance employee leaving a roller-coaster area was struck by a car at the lower track level, suffered serious injuries, and later died in hospital. The brief summary does not establish further causal findings, so it cannot show from its account alone how the employee reached the track level or what controls were operating. Read the OSHA incident record.
What the record teaches: a short incident summary is not a complete causal investigation. It documents a fatal collision involving a worker and a coaster car, but assigning responsibility to a specific maintenance practice, access control, or operating procedure would go beyond the published summary.

What these cases do—and do not—show about ride safety
The three events differ in the people harmed, the circumstances, and the detail available about cause. The Dallas alert says the cause was still under investigation when it was issued. OSHA’s Magic Mountain summary identifies the employee’s presence in a restricted area while the ride operated as the cause. The Great America record gives a brief event description but does not reconstruct the causal chain. Treating all three as examples of one common failure would erase those distinctions.
These records also concern unusual serious events, not the ordinary visitor experience. IAAPA publishes regional ride-safety reports based on defined reporting systems and datasets; those aggregate reports are a more appropriate place to look for incident patterns than a handful of selected accidents. Its index lists regional fixed-site reports, including 2024 reports, and identifies the 2024 North America fixed-site report as published in November 2025. IAAPA’s European Ride Safety Reporting System began in 2007 to collect incident information and report aggregate findings. How to read accident accounts responsibly
- Separate the outcome from the cause. A fatality or injury may be documented even when the mechanism or contributing factors remain unknown.
- Check whose safety is involved. An employee working near an operating ride faces different circumstances from a guest riding it; these records should not be blended into a single visitor-risk claim.
- Look for scope and method. A government review of selected investigated incidents, a historical national dataset, and an industry report are not interchangeable sources.
- Require evidence for claimed reforms. A serious accident can prompt scrutiny, but an accident record alone does not prove that a lasting rule, design, or operating change followed.
IAAPA frames its own position this way: “One injury is one too many and the industry is continuously working to enhance all aspects of safety.” That is the association’s statement of principle, not a finding about any of the three incidents above. IAAPA’s ride-safety reports page.
