Demi Moore anchors it as former star Elisabeth Sparkle; Margaret Qualley plays Sue, her younger double. Their uneasy arrangement gives the film a sharp premise—and a route to increasingly graphic excess. Calling this Moore’s best work is a judgment, not a consensus, but her performance is the film’s emotional center.
What is The Substance about?
Elisabeth Sparkle, once a star and now the host of a fitness show, is fired by studio head Harvey. She encounters a mysterious substance that creates Sue, a younger version of herself. The two must alternate: “One week for you. One week for the new you,” as the Festival de Cannes synopsis puts it. That rule turns rejuvenation into a contest over time, attention and control. The film’s conceit is fantastical; its target is familiar pressure to treat youth as a prerequisite for being seen.

Fargeat wrote and directed the 2024 film, which Cannes selected for competition and awarded Best Screenplay. Cannes credits Moore as Elisabeth, Qualley as Sue, and Dennis Quaid as Harvey. Why Moore’s performance makes the satire land
Moore plays Elisabeth as someone caught between public confidence and private vulnerability. The character’s predicament is not simply that she wants to look young; the world around her has made visibility feel conditional on it. Moore gives that pressure a human scale, grounding a story that steadily grows less realistic. Qualley, meanwhile, makes Sue more than a cosmetic upgrade: she is both the fantasy of starting over and a rival presence with her own force.

MUBI praises Moore’s performance and describes the film as turning “toxic beauty culture inside out.” That is a useful description of the film’s central device: it makes the wish to improve oneself literal, then exposes the violence embedded in treating a person as a project to be replaced. Does the outrageous escalation work?
The film’s most divisive choice is also its most distinctive: it pushes the premise far beyond restrained psychological horror. TIME critic Stephanie Zacharek’s May 21, 2024 review focuses on the film’s grasp of insecurity and aging, while its headline warns that it can “spin out of control.” Those reactions frame the question more accurately than a simple choice between bold and excessive: does the grotesque climax deliver catharsis, or does spectacle overtake the character drama?

For AP, Krysta Fauria described the shift this way: “what begins as a dread-inducing but still relatively palatable sci-fi flick spirals deeper into absurdism and violence, eventually erupting — quite literally — into a full-blown monster movie.” The escalation is part of the argument, not a late detour into subtlety. How gory is The Substance?
Very. Variance Films rates the film R for strong bloody violent content, gore, graphic nudity and language. This is not a spoiler about a single scene; the grotesque physicality is central to the film’s body-horror approach. Is The Substance worth watching?
It is a strong pick for viewers who want horror with an argument behind its shocks: Moore’s performance gives the satire a personal cost, while Fargeat’s formal excess makes the film hard to mistake for a conventional drama. It is a less safe recommendation for anyone seeking restrained horror, understated satire or a gore-light watch. The film’s real gamble is whether its increasingly extreme form sharpens its critique of beauty culture or swallows it.
