The Substance Ending Explained: What Monstro Elisasue Means

Elisabeth’s transformation into Monstro Elisasue ends in public rejection and violence. Here’s what Coralie Fargeat has said about the finale’s meaning.

Spoilers for the ending of The Substance. Elisabeth’s final transformation into Monstro Elisasue brings the film’s private body horror onto a public stage: the crowd rejects her, and the creature erupts into bloodshed. Writer-director Coralie Fargeat says the creature wants to be loved for who she is, not for her appearance, and that the explosion is directed at an audience representing the world. The finale is both a grotesque plot climax and a pointed critique of beauty, control, and the violence women face.

What happens at the end of The Substance?

Elisabeth Sparkle, a former television fitness star played by Demi Moore, uses the fictional Substance to generate Sue, a younger version played by Margaret Qualley. The treatment requires them to alternate bodies every seven days, a rule the official Festival de Cannes film record describes as a “perfect balance”; the US distributor likewise says there are no exceptions.

The arrangement deteriorates as Elisabeth and Sue struggle over the life and attention they share. At the climax, Elisabeth appears in a grotesque hybrid form, Monstro Elisasue, hoping to reclaim a place in public view. The audience rejects her. The creature’s body then becomes the source of escalating violence, turning the public appearance into a blood-soaked catastrophe.

Why does Elisabeth become Monstro Elisasue?

On the level of the story, the transformation is the physical culmination of the conflict between Elisabeth and Sue and the consequences of their broken balance. It is not a realistic medical outcome: the Substance is a fictional device, not a procedure or treatment the film presents as medically possible.

Symbolically, Monstro Elisasue embodies the cost of trying to satisfy incompatible demands: remain visible and desirable, but also maintain control over a body that changes and ages. Elisabeth’s monstrous appearance does not win the acceptance she wants. Instead, the crowd’s response makes the public gaze part of the horror.

What does the final explosion mean?

Fargeat directly connects the creature’s rejection to a desire for recognition beyond looks. In an Inverse interview, she says, “She wants to be loved for who she is, not what she looks like.” She describes the explosion as something that “explodes in the face of the audience, which represents the world.” The image turns the violence outward: the world that judges women’s bodies is made to confront the spectacle it helped create.

Fargeat also says, “We’re all part of having shaped such a violent world for women.” That is her stated interpretation, not a requirement that every viewer read the ending in exactly the same way. The final eruption can be understood at once as the brutal consequence of Elisabeth’s story and as an accusation aimed at the culture surrounding her.

How do the film’s settings reinforce that reading?

Fargeat has described the film as fundamentally about “the violence of control” in an interview with Vogue. She explains that the empty white bathroom works as an abstract, cocoon-like mental space where Elisabeth confronts herself, rather than as an ordinary, realistic location. She also describes the orange television corridor as marking the trajectory of Elisabeth’s life.

In a Vanity Fair scene breakdown, Fargeat says symbolism is central to how she writes and directs, and that “I wanted the movie to be about women’s bodies”. The film’s extreme transformations and settings therefore work as visual argument as well as plot events: they make the pressures around women’s bodies impossible to treat as invisible or polite.

What is the film, and who made it?

The Substance is a 2024 feature written and directed by Coralie Fargeat. The Festival de Cannes lists a 140-minute runtime, identifies Demi Moore as Elisabeth Sparkle, Margaret Qualley as Sue, and Dennis Quaid as Harvey, and records the film’s 2024 Best Screenplay award. Its synopsis and the US distributor’s page establish the alternating-week premise that drives the ending.
