The Most Devastating Injuries in WWE History

A comparison of devastating WWE injuries, from Darren Drozdov’s permanent paralysis to the career-threatening cases of Edge, Shawn Michaels, Steve Austin, and Triple H.

By lasting physical consequences and impact on a wrestler’s career, Darren Drozdov’s 1999 ring accident stands out as the worst injury in WWE history: his family said it left him quadriplegic and unable to move from the neck down. Other defining cases include Edge’s medically advised retirement, Shawn Michaels’s career-threatening back injury, and Steve Austin’s broken neck. This is an editorial comparison, not an official WWE ranking; the cases below are ordered by lasting impact and career consequences.

How these injuries are compared

There is no single objective scale for ranking wrestling injuries. This article weighs three factors: lasting physical consequences, whether an injury ended or threatened an in-ring career, and the interruption and recovery that followed. WWE’s accounts document several major cases, but they do not establish an exhaustive list or an official severity ranking.

Injuries with the most lasting consequences

Darren Drozdov: a life-altering ring accident

In a 2023 notice announcing Darren Drozdov’s death, WWE reproduced a statement from his family. They said a ring accident in 1999 left him quadriplegic and that he had been unable to move from the neck down for 24 years. That description comes from his family’s statement, rather than a published medical record. The family also wrote: “There is always another day. Just because I’m paralyzed and stuck in a wheelchair, doesn’t mean my life is over.” Read WWE’s notice and the family statement.

Injuries that ended or threatened careers

Edge: spinal risk led to retirement in 2011

On April 11, 2011, WWE reported that Edge was retiring after an MRI review. WWE said he had undergone a two-level spinal fusion involving the discs between C5, C6, and C7 in 2003. After he returned to competition, stenosis developed above and below the fused discs. According to WWE’s account of the 2011 decision, his doctors would not clear him to wrestle again because continued competition could risk paralysis or death. Shawn Michaels: a back injury and a four-year absence

During his 1998 Royal Rumble casket match against The Undertaker, Michaels landed on the edge of the casket and injured his lower back. WWE later reported that he suffered two herniated discs and a crushed disc. He continued wrestling for roughly two months, including his WrestleMania XIV match, then underwent career-threatening surgery and retired from active competition. Steve Austin: a broken neck after Owen Hart’s piledriver

At SummerSlam 1997, Owen Hart’s piledriver during an Intercontinental Championship match drove Austin’s head into the mat. WWE says Austin broke his neck and experienced temporary paralysis. He regained feeling, finished the match, and was helped backstage. The injury kept him out of ring action for months and affected him through the rest of his career; he later had neck surgery and effectively retired after WrestleMania XIX in 2003. Triple H: a complete quadriceps tear

WWE dates Triple H’s injury to May 21, 2001, during a tag-team match. He completely tore his quadriceps while intervening to save partner Steve Austin from Chris Jericho’s Walls of Jericho. WWE described the injury as debilitating and a threat to end his career prematurely. Its account does not give a complete return-to-ring timeline. Read WWE’s retrospective account.

Mick Foley’s Hell in a Cell fall

Foley’s 1998 Hell in a Cell fall is one of WWE’s defining and punishing match moments. WWE’s oral history collects recollections from Foley, Jim Ross, referee Tim White, and others who were there. A separate Michaels injury: the 2007 concussion report

WWE reported in 2007 that Michaels suffered a severe concussion at Judgment Day, his second concussion in three weeks. The report quoted physician Ferdinand Rios expressing concern about repeated head injuries and said Michaels was sent home for evaluation. This is a dated account of that episode, not current medical advice, and it is separate from his 1998 back injury. Read WWE’s 2007 report.

So, what was the worst injury in WWE history?

On the comparison used here, Drozdov’s injury is the most devastating because the reported consequences were permanent and life-altering. Edge’s case stands out for the risk that ended his career; Michaels’s and Austin’s injuries were career-threatening, with lasting effects described in WWE’s accounts. Foley’s fall belongs among wrestling’s most infamous dangerous moments, but the oral history reviewed does not provide the clinical detail needed to rank it as a diagnosed injury alongside them.
