In “Endure and Survive,” Joel and Ellie’s uneasy alliance with Henry and Sam is cut short by a devastating infected attack and an even more devastating loss. The episode, written by Craig Mazin and directed by Jeremy Webb, was released early on February 10, 2023, ahead of the Super Bowl.
Why Kathleen is hunting Henry
The episode opens in Kansas City after the resistance has overthrown FEDRA. Kathleen, the resistance’s leader, is searching for Henry. Her pursuit is personal: Henry gave up Kathleen’s brother to FEDRA in exchange for medicine that could help his younger brother, Sam, who had leukemia. Henry’s choice was an act of protection; to Kathleen, it helped bring about her brother’s death. The result is a conflict shaped by grief on both sides, not simply a fight between a hero and a villain.
In the present, Joel and Ellie are still trapped in the city when Henry and Sam find them. Henry proposes a way out. Joel distrusts him, but accepts the alliance because cooperation offers their best chance to escape. Henry’s caution is constant: his decisions revolve around keeping Sam safe.
How the infected attack changes the escape
Kathleen’s forces attack the group, and the gunfire and fighting draw infected up from underground. Clickers are among them, along with a Bloater—a particularly imposing infected presence. The resistance is overwhelmed. Kathleen and her lieutenant Perry are killed in the chaos, giving Joel, Ellie, Henry, and Sam a chance to get away.

#1 Best Overall
The outbreak creates an opening, but not a clean escape. Sam has been bitten. Ellie tries to help him, but the group cannot prevent what follows.
What happens to Henry and Sam?
Sam turns overnight. Henry shoots him, then kills himself. Joel and Ellie bury the brothers and continue west without them. The speed of the ending makes the earlier scenes of Henry’s vigilance and Ellie’s growing closeness to Sam especially painful: the danger they were trying to outrun reaches them anyway.
Why the tragedy matters to Joel and Ellie
Henry and Kathleen are linked by their devotion to family, but that devotion takes different forms. Henry’s decisions are driven by the effort to protect Sam; Kathleen’s grief has narrowed into revenge. HBO’s companion podcast frames its discussion of the episode around parental love, a theme that connects the two characters without making their choices equivalent.
Ellie’s attempt to help Sam also sharpens the story’s questions about her immunity. She is close enough to him to want to save him, but her effort cannot undo his infection. The episode makes clear that being immune does not mean she can protect everyone she cares about.

How the show changes Sam from the game
In the television adaptation, Sam is deaf. That is a specific change to his characterization and communication, and showrunners Craig Mazin and Neil Druckmann discuss the decision on HBO’s official companion podcast. Their episode conversation also covers casting and the infected, including Bloaters.
The adaptation comparison is most useful when kept to what the available episode discussion establishes: Sam’s deafness changes how he communicates, and Henry’s protectiveness remains central to their relationship. Kathleen’s expanded role and the Kansas City resistance provide a broader context for the pursuit. The infected attack, meanwhile, turns the group’s escape into a chaotic break from the besieged city. Avoid treating every beat as a direct game-to-show match; the sources establish these comparison points, not a scene-by-scene account.
