Abraham Verghese’s novel The Covenant of Water is a multi-generational family saga set in Kerala, South India, covering 1900 to 1977. Its central mystery is a drowning: according to the publisher’s description and the author’s own interview, at least one member of the family dies by drowning in each generation, and the story follows three generations as they search for an explanation. The book asks how love, faith and medicine respond to that inheritance, and where the line between the observable and the unexplained falls.

What the novel covers

The book is published by Grove Atlantic as a novel. Grove describes it as an epic of love, faith and medicine, and the story moves across roughly seven decades, from 1900 to 1977, as stated in the Poets & Writers author Q&A and the Harvard Book Store catalog entry. The setting is Kerala, with the Malabar Coast featuring in the Poets & Writers description. The Harvard Book Store listing also identifies the family as Christian.

Because the story is organized around three generations rather than a single protagonist, readers should expect the narrative to shift between family members and periods. The family history is the spine of the book; the recurring deaths are the question that links the generations.

The recurring drowning

The novel’s driving mystery is a pattern: a member of the family dies by drowning in each generation. Both the publisher’s premise and the author interview describe this pattern as the reason the family is pursuing answers. The novel does not present the drownings as a single event to be solved in one plot line. Instead, each generation confronts the same affliction, and the family’s attempts to understand it shape how its members relate to one another and to their faith.

Where medicine fits

Verghese is a physician as well as a novelist, and in a 2023 Stanford Medicine article he describes the book’s major themes as love and faith. In his account, medicine is the vehicle through which the novel explores those themes rather than the subject itself. That framing helps explain why the book is as interested in how people respond to illness, loss and uncertainty as in any single diagnosis.

For readers approaching the novel through its medical strand, the most useful question is not whether a condition can be named, but how characters use clinical knowledge alongside faith and family explanation when the cause remains unclear.

A reading guide: two axes to follow

The publisher’s premise and Verghese’s comments suggest two lines of attention that can organize a reading:

  • The multi-generational family mystery. Track how each generation’s drowning is described, what the family knows about earlier deaths, and how the search for an explanation passes from one generation to the next.
  • Medical and historical strands. Notice how medical care, belief and the historical setting between 1900 and 1977 shape the characters’ choices and their interpretation of the deaths.

These are interpretive suggestions rather than a settled reading. Faith and love are the themes the publisher and author emphasize, so they are a natural lens for asking how the characters respond to the recurring losses.

Art and the real and unreal

The title places art and medicine, and the real and the unreal, in the same phrase. The sources available for this article describe the novel’s premise and Verghese’s stated intent, but they do not establish a single authoritative interpretation of how the novel treats art or the boundary between the real and the unreal. Readers who want to explore those strands should treat them as open questions to test against the text, rather than as conclusions a critic or the author has settled.
